What You'll Be Doing

The Patient Liaison (PL) participates in the intake process of new and re-engaged clients into the Ryan White Program. The PL assists Medical Case Managers (MCMs) in educating newly diagnosed patients entering the HIV/AIDS continuum of care to navigate established systems and programs such as ADDP or Charity Care to ensure patients have prompt access to medical and social services and medications.

The PL performs testing as an important component of high impact prevention. The PL is responsible for the entry of required clinical and programmatic data into the program's EMR and the federal Ryan White's CAREWare platform. In addition, the PL assists the QI Coordinator, the Program Director and the Clinical Services Coordinator with clinical flow, quality management initiatives and other administrative tasks.
