Description

  • The position will require travel to the following hospitals within the Northwestern Medicine network: Kishwaukee, Delnor and Central DuPage Hospital*

Provides diabetes and nutrition education to patients, staff, and the community. Develops and provides individualized diabetes and nutrition education to patients and their families/caregivers.

Assesses patient understanding and cooperation with integrating nutritional planning into the patient's lifestyle. Modifies plans to promote success. Collaborates with the healthcare team to integrate nutrition therapy into the management of diabetic patients care.

Develops and implements educational activities for staff and the community. AA/EOE

Qualifications

Required:

  • Bachelors degree in nursing.

  • Licensed Registered Nurse (RN).

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

  • Must be able to work in a stressful environment and take appropriate action.

Ready to join our quest for better? Description Collaborates with the director of care coordination to oversee the discharge planning process and operations of the case management team. Serves in a lead role by utilizing communication skills to build trusting interdisciplinary relationships; engaging in conflict management, coaching, mentoring, and staff development; fosters an environment for shared decision making. Collaborates with the director of care coordination to provide the case management team with an environment that supports their professional practice, health and well-being. Coaches and mentors and develops case managers in their professional practice. Collaborates with director and department secretary to ensure appropriate and complete orientation of new staff. Identifies appropriate preceptor(s) and assigns schedule to new staff. Collaborations with case managers to ensure resources are coordinated to meet patient needs. Partners with physician to build a collaborative inter-professional practice environment. Contrib Qualifications Required: Graduate of an accredited school of nursing with a BSN. OR Masters Degree in Social Work from a CSWE accredited school of social work & Licensed Clinical Social Worker (LCSW) Minimum of three years of case management experience in an acute care setting or health care environment.
