Patient Centered Care Consults
1 for each location: Hartford-CT, Manchester-NH, and Atlanta-GA
The Patient Centered Care Consults are responsible for providing support to providers to improvethe effectiveness and efficiencies of provider practices.
Primary duties may include, but are not limited to:
Obtain and analyze quality metrics and report for careopportunities, support practice implementation of Quality improvementopportunities, identify action plans for providers to implement to improvecost, quality and the patient experience.
Actively participate in developing programrecommendations and action plans (Internal).
Support the efforts of providers and practice teams onoffice based care delivery interventions resulting in cost of care savings andimproved health outcomes for patients.
Engage with network relations representatives to meetprovider and practice service needs. Provide education for practices todevelop expertise with metrics and data review for quality improvement.
May participate in various community and multi-departmentcommittees to assure effective program implementation.
Track progress of program and prepare status reports tomanagement or senior management and providers.
Maintain up to date knowledge on Patient Centeredmedical Home, Care Delivery System Redesign, Accountable Care Organizations andHEDIS.
Requires a BA/BS; 3 years health care experience; or anycombination of education and experience, which would provide an equivalentbackground.
Physician environment experience in practice transformation/qualityimprovement, ambulatory care setting quality and efficiency metrics, andelectronic health records preferred.
