Job Summary: Assures and/or provides patient care in accordance with the RN job description.

Serves as a responsible and accountable clinical expert, role model, educator and resource, who facilitates professional development of staff. Collaborates with Director and Patient Care Manager to assure competent staff performance to promote quality patient outcomes.

Minimum Requirements:

Current PA RN license. BSN Three years recent clinical practice experience. Demonstrated leadership and teaching abilities.

Active member of a professional nursing organization. Demonstrates initiative , functions independently as well as collabboratively. Demonstrates ability to initiate, accept and adapt to change.

Demonstrates time management and organizational skills.

Preferred Qualifications:
MSN or Masters in education/nursing or a health care related field.

Prior teaching experience in healthcare. Licensure and Certifications: Current Pennsylvania Registered Nurse licensure.

Current BLS Health Care Provider. BLS required;ONS Chemo/Biotherapy certification;interpersonal skills and emotional intelligence.;PA RN Licensure;OCN or OCN certification required within 2 years of eligibility;Active membership in professional nursing organization(s) required;Ability to work independently;Good communication skill;Bachelor#s degree in nursing Demonstrated success within complex health care environments.Excellent organizational;customer service skills;Master#s degree in education/nursing clinical specialty/health care related field.;strong computer skills;Strong communication skills #

VIEW JOBS 10/18/2019 12:00:00 AM 2020-01-16T00:00 Responsibilities: Under the supervision of the Wellness Center Administrator, this position is responsible for providing select program interventions according to established health management program guidelines and the member population. The Care Connector functions with direction from program's Case Manager or Supervisor/Manager and is responsible for screening identified members based on plan stratification for the program. Interacts with members, providers and other staff to implement program interventions, document activities, assist providers in the authorization process and refer risk appropriate members to professional staff according to protocols. Prior healthcare experiences a plus. Principal Accountabilities: * Supports the daily operations of the RROT through interaction with staff, facilities, vendors and providers. * Maintains a current knowledge base of Rapid Response processes and timeliness. * Uses good listening skills, conducts outreach calls, collects data according to script, tools, and protocols meeting both productivity and performance expectations as identified by unit supervisor/or designees. Conducts all calls in a courteous and customer service friendly manner. Refers as appropriate when indicated by workflow. * Creates, updates, maintains and/or closes authorizations or tasks for services as assigned within process guidelines. Routes case to appropriate associates based on established guidelines. * Processes all incoming and outgoing correspondences/faxes in accordance with required standards and within respective timeliness guidelines. Refers to the appropriate clinical team members for review as defined by workflow. * Performs in a call center environment appropriately processing or triaging calls from members and providers. * Communicates directly with members to identify needs, and provide information on health care access and preventative health interventions and screening. * Clerical responsibilities such as processing urgent scanning, mailing requests and document retrieval. * Demonstrates a professional and courteous manner when communicating with others with the ability to clearly and accurately state the agreed upon resolution. * Adheres to AmeriHealth Caritas FoC Policies and Procedures, process standards, Standard Operating Procedures and maintains current knowledge of member benefits, rights and responsibilities. * Performs other related duties and projects as assigned within the assigned timeframes. * Adheres to AmeriHealth Caritas Family of Companies policies and procedures. * Supports and carries out the Caritas Mission & Values. * Complies with AmeriHealth Caritas FoC and HIPPA confidentiality standards to protect the confidentiality of member information. Education/Experience: * High School/GED * 1-3 years experience * Proficient PC skills in a Windows based environment including word processing, spread sheets and working in database programs. * Working knowledge of Plan Benefits and Services is preferred. * Proven ability to keep accurate and timely records and documentation according to established processes. * Effective oral and written communication skills. * Excellent customer service and interpersonal skills on phone and in-person. * Ability to handle multiple tasks simultaneously.
