PAY RANGE: Band 2 ($18.00 - $25.00/hour)

The mission of Charles River Community Health (CRCH) is to partner with individuals and families so they can thrive and lead healthier lives by delivering the comprehensive, integrated, and equitable primary healthcare that matters most to them, the Patient Care Navigator will work with a dynamic, dedicated care team to provide quality primary care services in our health care center. The Patient Care Navigator position is integral in identifying gaps in preventative care services and ensuring follow up for chronic diseases and care coordination.

YOUR RESPONSIBILITIES

  • Ensure all patients are checked out after their medical visit. This includes but is not limited to identifying preventative care which is overdue, scheduling mammograms, booking follow up appointments that are within 2 weeks, entering follow up appointments into the EMR if beyond two weeks, ordering in-house referrals and providing end of visit summary as well as educational materials as requested by the primary care provider.

  • Follow up patient eligibility, scheduling the appointment, request previous records from outside facilities, and work with the providers and Prevention and Wellness Department to place the order.

  • Follow up with providers appointment request.

  • Patient contact via in person, phone or e-mail.

  • Manage care coordination, ensuring appropriate follow up for all urgent care, hospital discharges, and emergency room visits as notified by alerts. This includes but is not limited to requesting medical records as well as communicating with nursing and primary care provider teams.

  • Review faxes and distribute to appropriate team member bin for processing.

  • Document all calls and patient contact in the medical record according to established protocols.

  • Participate in appropriate training/workshops to ensure competence.

  • Meet bi-weekly with Customer Service Coordinator to review workflows and identify areas for improvement

  • Work collaboratively with other Health Center staff in meeting accreditation and funding requirements.

  • Attend Medical Home and/or other training or meetings as required.

  • Perform other duties as directed.

  • Work in both locations, Brighton and Waltham

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

  • Previous experience in medical field preferred.

  • Mature, responsible, team orientated individual with ability to multi-task.

  • Must be Bilingual in English and Spanish (speak, write, and understand).

  • Experience in use of computer systems, EMR preferred.

  • Experience in use of computer systems preferred.
