Heartland Hospice a part of HCR ManorCare that provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy is currently searching for an experienced RN to fulfill the role as a Patient Care Manager in their Northbrook hospice location.

The Patient Care Manager is responsible for the supervision, orientation, instruction and on-going evaluation of all patient care staff who are engaged in the delivery of care. The PatientCare Manager assures compliance with agency policies and procedures, state and federal laws, regulations and accreditation standards.

Qualifications

Minimum of one year experience in home health care, or hospice nursing preferred, may be specified by state.

Previous management or supervisory experience preferred.

Working knowledge of Home Care or Hospice Conditions of Participation

Educational Requirements

Currently licensed as a Registered Nurse (RN) in good standing with the State Board in which he/she practices.

Purpose & Scope: Leads and directs the talent acquisition functions for the Americas region in support of the Global HR strategy to attract, develop and retain the talent necessary to execute on our business strategy now and in the future. Manages the Astellas Talent Acquisition team and is accountable for managing the third-party partners supporting talent. Responsible for partnering with managers and employees at all levels within the company to identify talent gaps, conduct needs assessment and structure/implement innovative staffing solutions with a focus on expanding the quality and diversity of the Astellas workforce. Ensures alignment with the global Talent strategy whilst at the same time ensuring local requirements and legislation is followed. This position reports to the Global Practice Lead for Talent Acquisition. Essential Job Responsibilities: * Provides vision, leadership, planning and management for the strategic talent acquisition programs. As the leader of the Talent Acquisition Team, collaborates to develop, implement, manage and analyze policies and procedures for recruitment, selection and job placement. Partners with management teams to ensure that staffing goals are aligned with business needs. Manages talent acquisition activities across the US enterprise. This includes managing external vendors who support the talent acquisition process * Manages third party partners and developing strategies to ensure high quality, service and value * Ensures talent acquisition solutions are aligned with the business, working across the function, reporting into the Global Practice Lead to assist in the creation of a global TA service * Works with the HRBP's and business leaders within the region to ensure service levels are met, identifying risks, scoping ad hoc recruitment projects and delivering a best in class talent acquisition solution for Astellas * Regularly reviews and refines processes in line with regulations, business needs and identifying key talent gaps where innovative solutions may be required * Keeps fully abreast of developments in the Talent Acquisition space and acts as a TA expert within the region * Works cross functionally to develop areas such as D&I and social media in support of the global Talent Strategy * Works with the talent team to review and refine selection and assessment tools and criteria * Works cross functionally to review and refine processes with regards to pre-boarding and on-boarding to ensure best in class hiring manager and candidate experience * Operates as the Escalation point for all TA issues within the region * Partners with the HRBP community help to translate the Strategic Workforce Plan and Succession Planning into a talent acquisition strategy for the region * Participates in the candidate interview and selection process as needed for Senior level leadership positions * Leads special projects as needed and may have some matrix line management responsibilities Quantitative Dimensions: In addition to the responsibilities outlined above, this position is responsible for the Talent Acquisition needs of the organization across all Americans Region. Organizational Context: This position reports to the Sr. Director Talent Acquisition and has 4+ direct reports.
