Job Summary:# Hospital based Case Manager coordinates and manages patient care to ensure continuity of care delivered with compassion, excellence, and reliability. The Case Manager collaborates with the physicians to evaluate, prepare, educate, and refer patients for the next level of care and works closely with ancillary services to assist in arranging placement in the appropriate care setting as well as coordinates a safe and timely discharge plan.# The Case Manager performs case management through direct referrals and reviews of medical records.#Facilitates and participates in organizational quality improvement activities including treatment plan assessments, discharge planning, and coordination of care.



Minimum Requirements:


Bachelor of Science in Nursing preferred, together with a current R.N. license to practice nursing in the State of Michigan.# Previous experience in Case Management preferred.# Minimally, 3-5 years progressive experience in a clinical acute care setting with current experience relevant to patient population served. Individual must be detail oriented, have computer application experience as well as excellent written/oral communication skills.# Knowledge of Medicare, Medicaid and commercial payer regulations preferred# # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # ### Skills, Talents and Abilities:# Must be able to work cooperatively as a member of a team, able to communicate effectively with patients and their families as well to all levels of the hospital staff.# Must be flexible, self-motivated, interested in program development and capable of making independent decisions.# Ability to be non-judgmental and work with those holding diverse opinions, religions and cultural values.

Facilitates care coordination and ensures continuity of services in order to meet patients, family and staff needs. 4.##### Strong interpersonal communication skills:

Displays active listening and empathetic communication skills with patients, families and staff. Engages and collaborates with patients, families, multi-disciplinary staff and the community in care planning, delivery and discharge. Builds solid relationships, negotiates effectively and works well with individuals of all ages, backgrounds and socioeconomic position.

At least 1 year experience successfully working with the public with minimal supervision. Proven ability to communicate effectively with both public and co-workers orally and in writing, and posses the ability to work positively with all levels of clinical staff in completion of job responsibilities. Must be computer literate and posses a proven ability to effectively use multiple computer systems. Successful completion of a medical terminology course required within 6 months of hire date. Use Compliance Checker programs to screen for medical necessity and to determine need of advanced beneficiary notice (ABN). Determine co-pay and deductible information from insurance eligibility and coverage inquiries Demonstrate ability to accept payments from patients, record and issue receipts. Recognize self-pay accounts and collects payment or makes referral to appropriate Financial Counselor. Demonstrate ability to scan medical record chart and index appropriately. Independently identify accounts that need more attention and independently identify methods for resolving difficult accounts. Assist in the development and maintenance of departmental policies and procedures Communicate with Central Scheduling to adjust schedule changes � i.e., cancellations, no shows and re-scheduled visits and walk-ins.
