Program Description:

Our sexual and reproductive health centers, located in Fort Greene and Brownsville, provide affordable, comprehensive, and confidential reproductive and sexual healthcare to more than 4,000 people, including adolescents each year. We offer a complete range of services, including free walk-in pregnancy testing, gynecological exams, preconception care, prenatal care, birth control, emergency contraception, medication abortion, sexual and reproductive healthcare for all genders, teens' sexual healthcare, STI testing and treatment, HIV testing and counseling, mental health services, and health education.

Position Summary:

Under the direction of the Center Director (CD), the Patient Care Facilitator performs those components of the patient care regimen of the PHS Sexual and Reproductive Health Centers which may be delegated to paraprofessional nursing personnel.

Specifically, the Patient Care Facilitator will:

  • Direct patients to the appropriate center areas (which may involve escorting patients when indicated), and for facilitating a smooth flow of traffic.

  • Orient patients to SRH Center services including prenatal, family planning, and gyn services.

  • Prepare and assists with the examination and treatment of the patient, providing concurrent support and reassurance to the patient, and facilitating communication between the patient and the provider, including translation as appropriate.

  • Maintain supplies, including inventory, unpacking, and storage in the patient care areas and assists team in ordering new supplies when needed.

  • Sterilize and stores instruments and containers.

  • Ascertain that all pertinent information is included in the patient's record and inform the appropriate nursing staff regarding any information pertinent to the care of the patient.

  • Complete laboratory tests as requested by the medical provider including drawing blood, urine dipstick, pregnancy tests, hematocrit, Ora-Quick, and glucose challenge and tolerance tests.

  • Participate in appropriate learning experiences designed to maintain competence in the role and to enhance and enrich the job experiences.

  • Record laboratory tests processed and follow-up on labs received/not received in electronic medical record.

  • Perform and record quality control tests. Perform other related activities as shall be deemed appropriate by center management.

Qualifications and Skills:

  • Graduation from a senior high school or possession of a high school equivalency diploma issued by a local Board of Education or a State Education Department or possession of a G.E.D. Certificate.

  • One to two years of full-time experience as an assistant in a doctor's office or in a hospital or any other acceptable health service.

  • Phlebotomy skills

  • Bilingual English/Spanish preferred, not required.

  • Experience using an Electronic Medical Record (EMR).

  • Computer skills.

  • Must be willing to travel to various locations that are accessible via public transportation.

  • Salary: $38,000.00- $38,000.00.
