St. Vincent's Medical Center in Bridgeport has more than 3,200 employees. It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children. St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network and is the system's second-largest hospital.

There are ten (10) beds on this inpatient unit giving our patients access to ongoing acute medical management, 24-hour rehabilitative nursing care and the latest rehab techniques. JOB SUMMARY

HHC is seeking a dedicated and caring IRF-PAI Coordinator to be part of an experienced team that brings decades of experience in providing the most innovative care. This position will treat patients with all diagnoses with a focus on stroke, traumatic brain, or spinal cord injuries, as well as other neurological conditions. Our new unit will feature a collaborative interdisciplinary team atmosphere and approach to patient care. This is a great opportunity to work in a special inpatient rehab setting, while still having access to the opportunities and benefits available in a system with a full continuum of rehab care.

RESPONSIBILITIES

The IRF-PAI Coordinator reports to the Therapy Manager and coordinates the data collection on the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI), ensures the presence of supporting documentation, and provides staff education to ensure compliance with CMS guidelines. Coordinates all efforts for immediate issue resolution concerning Quality scoring. Conducts ongoing review of IRF-PAI related data, and actively collaborates with coding, nursing, therapy, and case management to ensure accurate and timely IRF-PAI completion. The IRF-PAI Coordinator also oversees the development of tools that assist in compliance and quality reporting for the IRU.

  • Associate degree required. Bachelor's degree preferred.

  • Minimum 2 years' experience in rehabilitation or closely related field required, inpatient rehabilitation specific experience highly preferred.

  • Experience with FIMs and IRF-PPS preferred.

LICENSURE / CERTIFICATION:

  • Current CT licensure in nursing or allied health profession required.

  • Strong analytical and critical thinking skills.

  • Detail-oriented, able to meet strict time frames.
