Patient Access Specialist - Inpatient (100% Fte, Day Shifts)

HARBORVIEW MEDICAL CENTER SOCIAL WORK has an outstanding opportunity available for a PATIENT ACCESS SPECIALIST - INPATIENT (SOCIAL WORK ASSISTANT 2).

WORK SCHEDULE

• 40 hours per week

• Day Shift

POSITION HIGHLIGHTS

• Provide financial guidance and care coordination to help affect early discharge

• Serve as a liaison between patient/family, HMC staff, and community agencies such as DSHS

• Assist with coordination and tracking of sponsored items and services

PRIMARY JOB RESPONSIBILITIES:

• Serve as an ambassador for the Care Management Department and promote effective working relationships

• Timely documentation of social work assistant activities and verbal communication to team members as appropriate.

• Comply with all aspects of HIPAA regulations.

• Maintain working knowledge of available community resources for continuing care options for medically and/or psychiatrically complex patients, to include Skilled Nursing Facilities, Intensive Inpatient Rehabilitation programs, and Adult Family Homes.

• Adhere to HMC/Patient Care Services /SW standards for documentation and confidentiality.

• Have basic knowledge of DSHS and Medicare benefit programs, eligibility requirements, relevant rules, and regulations.

• Provide notarial services for patients related to their medical admission or discharge planning.

REQUIRED POSITION QUALIFICATIONS:

Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.

OR

an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.
