Description:

Provides diagnostic and treatment interventions to assigned patient popuation to improve the patient's speech/language, oral/facial musculature and structure, cognitive skills, voice, fluency, breathing and/or swallowing abilities to maximum potential. The Speech Pathologist will provide this treatment per physician's order and coordinate therapeutic activities with the patient, caregiver and other health care disciplines.

The speech pathologist will work as part of the interdisciplinary team and follow the Kaiser and the department guidelines. Travels to various settings providing speech pathology services throughout the Northwest region as determined by the needs of the department.

Essential Responsibilities:

Evaluates patients communication/articulation, physical, cognitive, breathing, swallowing status, functional abilities and age specific needs of patients to determine the current level of functioning, self-care, self- responsibility, independence and quality of life.

Develops and implements a treatment plan which involves the patient, family, and social support system, Identifies and provides skilled interventions to reach achievable goals, coordinates and collaborates on treatment options, advocates enhancing patient's social support systems; facilitates appropriate environmental modifications. Provides medically necessary treatments based on established departmental guidelines and protocols; provides skills based on competency check lists.

Provides ongoing assessment and analysis of patient functioning and progress. This therapy involves providing treatment which takes into account the risks and benefits of that treatment regime relevant to the patient's medical condition, rehabilitation potential and prognosis. Travels to various settings providing speech pathology services throughout the northwest region as determined by the needs of the department.

Provides timely and complete documentation following department guidelines and compliance requirements. Follows any indicated HIPPA, Medicare, Medicaid, Joint Commission as well as state, organizational and department rules.

Coordinates therapy activities with other health care disciplines.

This includes coordinating services through consultation with appropriate agencies to ensure follow-up and continuing care as needed.

Participates in quality and performance improvement activities within the department including but not limited to measurement and continuous improvement of clinical competency, regulatory compliance, outcome achievement, resource management, work team performance and customer satisfaction. Performs other duties as required.

Education

Master's degree in speech pathology and completion of speech pathology patient training through Master's speech pathology program

License, Certification, Registration

Licensed in Speech Pathology in Oregon and Washington. Requires license in primary state, (Oregon or Washington) prior to first day of employment.

Application for licensure in secondary state must be made within five (5) weeks of start date. License in both states required within five (5) months of hire/transfer.

Certification of clinical competency from American Speech and Hearing Association required at time of hire/transfer.

Basic Life Support (BLS) for Health Care Providers required upon hire/transfer.

Valid Drivers License upon hire/transfer.

National Provider Identifier (NPI) required prior to employment start date.

Additional Requirements:

Demonstrates customer-focused service skills.

Preferred Qualifications:

Experience as a speech pathologist seeing an outpatient caseload.

Experience with treating dysphagic patients.

Experience with treating assigned patient population.

Experience treating pediatric patients

Experience in the evaluation and treatment of sensory feeding issues

Experience in the evaluation and treatment of speech, language, pragmatics, fluency, articulation, motor speech, voice, autism, and ACC

Ability to both evaluate and treat patients with dysphagia.

Ability to scope patients during a fiberoptic endoscopic evaluation of swallowing.

Ability to evaluate assigned patient population with specific skills in administering language, cognitive and articulation assessments.

Ability to treat assigned patient population with specific skills in treating patients with language, cognitive and communication deficits.

Ability to provide evaluation and treatment for patients with the following disorders: Dysphagia, voice disorder, communication/articulation problems, cognitive deficits.

Able to work autonomously and manage their own case load.

Excellent time management, problem solving and customer service skills.

Excellent written and verbal communication skills.

Operational knowledge or ability to learn computerized clinical documentation and office application systems.

This position will have competency and experience with assessment and intervention activities for special populations including, but not limited to, autism, pediatric dysphagia, extreme prematurity, congenital brain anomalies, Rett Syndome, etc.  The Speech/Language Therapist will evaluate each patient referred, establish effective treatment goals and objectives, develop and implement the treatment plan, providing family and caregiver training as needed, for successful carryover of strategies taught to improve developmental, speech, language, swallowing and cognitive skills of the patient/client.  Therapists are expected to supervise students and Clinical Fellows and to assist with other educational and research activities conducted by the department, division, and/ or DCH and OHSU hospital.  Please note that this is a Flex Therapist position.  Flex Staff employees will be called upon only when work is available.    Therapists evaluate each patient referred, establish treatment goals/objectives, develop and treatment plan and implement the plan.  Therapists are expected to supervise therapy interns, assist with other educational research activities conducted by the department or the hospital and meet hospital standards regarding documentation and productivity.  Job Requirements REQUIRED: * Masters degree in Speech/Language Pathology and possess a valid Oregon State license in Speech/Language Pathology; * OR, specifically when the position is a Clinical Fellowship Year position (CFY), meet the educational requirements, have an OR state conditional license, and be in the process of accumulating the supervised experience required for licensure (completion of Clinical Fellowship Year).  An Oregon State License in Speech/Language Pathology must be obtained within one year of appointment or employment will be terminated. * Current State of Oregon Speech Language Pathology License. * Current American Heart Healthcare Provider BLS CPR Certification * Competency and experience with assessment and interventions for special populations such as autism, pediatric dysphagia extreme prematurity, congenital brain anomalies, Rett Syndrome and other specialty pediatric developmental and rehabilitation patient populations. * Must have knowledge of inpatient medical environments and equipment. * Ability to respond quickly and appropriately to urgent medical situations that may arise with high risk or unusual patient populations. * Inpatient Hospital Environment * Ability to lift 25 lbs, to repeatedly carry equipment weighing more than 5 lbs and walk extensively on hard and soft surfaces. * Frequent bending and reaching to navigate medical equipment or perform treatment is required. * Able to perform the essential functions of the position with or without accommodation.
