JOB DESCRIPTION

BACKGROUND

Job Title: Part-Time Residential Youth Care Worker (Supported Apartments)

Reports To: Supported Apartment Supervisor

FLSA Status: Non-Exempt

PURPOSE OF JOB

To provide a safe and therapeutic environment for youth in the transitional living program

ESSENTIAL RESPONSIBILITY NO. 1 - 60% OF TIME

Description: Provide overnight support and supervision

Task No. 1 Ensure youth are in a Youth In Need apartment curfew

Task No. 2 Ensure youth are in their own apartments by the curfew indicated by their step

Task No. 3 Encourage youth to go to bed at a reasonable hour based on their work/school situation

Task No. 4 Monitor apartments for appropriate number of guests and guests leaving the apartment at the time indicated by the youths step

Task No. 5 Complete apartment checks every hour

Task No. 6 Ensure that youth remain in their apartment after curfew as needed

Task No. 7 Assist youth in watching for school cabs/buses in the morning

Task No. 8 Complete administrative tasks as assigned

ESSENTIAL RESPONSIBILITY NO. 2 10% OF TIME

Description: Provide safe, therapeutic environment for residents

Task No. 1 Facilitate evening activities in the staff apartment (dinner, game nights, etc.)

Task No. 2 Facilitate recreation activities

Task No. 3 Assist youth with daily activities (i.e. homework, laundry, etc)

Task No. 4 Model appropriate healthy behavior through professionalism

Task No. 5 Assist Case Manager in teaching life skills individually and in groups

Task No. 6 Provide case management assistance as needed (rides to doctor appointments, job interviews, etc.)

Task No. 7 Ensure youth are in a Youth In Need apartment at curfew and following other expectations of the Apartment Program.

Task No. 8 Ensure youth are being good neighbors.

Task No. 9 Attend, and provide rides for, evening activities (school extracurricular activities)

Task No. 10 Work in a collaborative, strength-based team approach

ESSENTIAL RESPONSIBILITY NO. 3 - 10% OF TIME

Description: Maintain communication with team to ensure quality services to residents

Task No. 1 Participate in change-over discussions at shift change

Task No. 2 Participate in weekly staffing

Task No. 3 Participate in weekly supervision with supervisor

Task No. 4 Communicate through expected documentation processes

Task No. 5 Inform Director, Therapist and other RYCWs of youths progress

ESSENTIAL RESPONSIBILITY NO. 4 - 10% OF TIME

Description: Assist resident in achieving goals towards independence

Task No. 1 Be familiar with each residents individual goals as stated in their transition plan

Task No. 2 Assist youth with identifying appropriate employment options

Task No. 3 Provide appropriate support for residents struggling with issues of independence

ESSENTIAL RESPONSIBILITY NO. 5 - 5% OF TIME

Description: Ensure all licensing requirements are met

Task No. 1 Attend required trainings in a timely manner

Task No. 2 Participate in additional training as requested to further professional development

Task No. 3 Maintain accurate record keeping and documentation

ADDITIONAL RESPONSIBILITIES - 5% OF TIME

Task No. 1 Must be able to assist with general cleaning and upkeep of house

FORMAL EDUCATION/CREDENTIALS (IF ANY) NORMALLY ASSOCIATED WITH JOB

Necessary: High School Diploma or GED

Desirable: Bachelors Degree in Social Work, Psychology or a related field

KNOWLEDGE

Understanding of the needs and challenges facing at-risk youth

Understanding of appropriate emotional and physical boundaries

EXPERIENCE

Experience working with at-risk youth is desirable

SKILLS

Conflict resolution

Intervention

ENVIRONMENTAL CONDITIONS

In door, home environment

Travel for various resident reasons (appointments, etc.) and recreational outings

PHYSICAL REQUIREMENTS

Must be physically capable of climbing stairs for resident supervision purposes

Must be able to drive large vehicle for transportation of resident purposes

DRIVING REQUIREMENTS

Must obtain Class E license

OTHER REQUIREMENTS

Primary schedule will be Sunday and Monday 10am

  • 3pm and Thursday and Friday 6pm

  • 11pm) with the exception of staff meetings and training

Position requires working on holidays

Must be insurable under YINs auto liability coverage

VIEW JOBS 1/13/2019 12:00:00 AM 2019-04-13T00:00 YOUTH IN NEED JOB DESCRIPTION BACKGROUND Job Title: Supported Apartment Program Case Manager Reports To: Regional Director RHY East FLSA Status: Non-Exempt PURPOSE OF JOB Assist youth in becoming productive members of society by collaborating with internal and external partners for care and development of overall wellbeing. Teach youth the necessary skills for long-term independence. ESSENTIAL RESPONSIBILITY NO. 1 - 65 % OF TIME Description: Teach youth basic life skills in the areas of hygiene, effective communication, resume writing, interviewing, banking, conflict resolution, cooking, cleaning, parenting, social skills, boundaries, career and professional development, etc. Complete case management activates with youth including applying for Medicaid/SSI, obtaining identification, scheduling and attending medical appointments, etc. Task No. 1 Carry a caseload of assigned youth Task No. 2 Meet 1:1 with youth 1-2 hours per week to assess and develop life skills Task No. 3 Coordinate and facilitate 1-2 weekly life skills groups for residential youth. Task No. 4 Organize monthly Group Home/Apartment gatherings to include guest speakers, job fairs, cook-offs and themed dinners. Task No. 5 Schedule and coordinate experiential field trips to coincide with life skills curriculum. Task No. 6 Identify and provide service learning opportunities for youth. Task No. 7 Assist youth in obtaining employment by assisting with job hunt, giving feedback and providing support and advocacy around vocational issues. Task No. 8 Assist youth in enrollment in an educational program and monitor progress Task No. 9 Assist youth in managing finances, creating a budget and developing a savings plan Task No. 10 Link youth with needed community resources Task No. 11 Assess youth for substance abuse and mental health services Task No. 12 Provide transportation as needed for appointments and meetings Task No. 13 Complete unannounced apartment visits with youth at least twice a month ESSENTIAL RESPNSIBILITY NO. 2 - 10% OF TIME Description: Case management and service plan coordination. Task No. 1 Utilize curriculum and develop additional activities for a comprehensive life skills program. Task No. 2 Develop service plans in coordination with Clinical Case Manager. ESSENTIAL RESPONSIBILITY NO. 3 - 5% OF TIME Description: Network with community resources Task No. 1 Establish mentoring network consisting of a variety of occupations and link youth with opportunities for professional mentoring. Task No. 2 Cultivate relationships with area community resources and maintain relationships with youths employers, landlords, school personnel, etc. Task No. 3 Network with community resources to establish new employment/partnership opportunities for youth and build on existing partnerships. ESSENTIAL RESPONSIBILITY NO. 4 - 15% OF TIME Description: Monitoring progress and outcomes of youth Task No. 1 Assess TLP/ILP youth using formal assessment tools and track progress Task No. 2 Attend weekly staffing to report on youths progress and partner with RYCWs and CCM to determine ways to assess and implement life skills in the group home and apartment setting. Task No. 3 Attend Family Support Team meetings, IEPs and any other related meetings to discuss youths progress in the program and transitional goals. Task No. 4 Monitor and document youths attendance and progress in life skills groups Task No. 5 Complete monthly progress reports and outcome reports. Task No. 6 Complete weekly documentation of meetings with youth in data management system Task No. 7 Reassess youth at minimum of every 3 months and track outcomes. ADDITIONAL RESPONSIBILITIES - 5 % OF TIME Task No. 1 Other duties as assigned.
