Qualification:

RN licensed in the State of Idaho

Valid Driver's License

Home Health and Hospice experience preferred

Essential Job Functions/Responsiblities

Patient Care
1.Completes an initial assessment of patient and family to determine home care needs. Provides a complete comprehensive assessment and history of current and previous illness(es).
2.Regularly re-evaluates patient nursing needs.
3.Initiates the plan of care and makes necessary revisions as patient status and needs change.
4.Uses health assessment data to determine nursing diagnosis.
5.Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
6.Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
7.Counsels the patient and family in meeting nursing and related needs.
8.Provides health care instructions to the patient as appropriate per assessment and plan of care.
9.Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
10. Acts as Case Manager when assigned by Clinical Supervisor or designee and assumes responsibility to coordinate patient care for assigned caseload.
11. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.

Communication
1.Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
2.Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
3.Communicates with community health related persons to coordinate the care plan.

Additional Duties
1.Participates in on-call duties as defined by the on-call policy.
2.Ensures that arrangements for equipment and other necessary items and services are available.

Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required. 2. Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. 3. Maintains clinical records on all patients referred to social work. 4. Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs. 5. Provides information to patients and families/caregivers and community agencies. 6. Serves as liaison between patients and families/caregivers and community agencies. 7. Maintains collaborative relationships with organization personnel to support patient care. 8. Maintains and develops contracts with public and private agencies as resources for patient and personnel. 9. Maintains and assists the volunteer coordinator in establishing a need for volunteer services with the patient and family/caregiver and entire hospice team. 10. Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings. 11. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 12. Assists physician and other team members in understanding significant social and emotion¬all factors related to health problems and death/dying issues. 13. Actively participates in quality assessment performance improvement teams and activities. 14. Other duties as delegated by the Clinical Supervisor .
