Essential Functions:

  • Develop and maintain a service relationship with youth and family, and help to design and implement a service plan

  • Provide youth with goals that help him/her to improve relationships with others and within the community

  • Provide parents with information on disciplining, behavior management, household management and communication and relationship enhancement when needed.

  • Works with the parents/caregivers so that they can support youth in achieving goals

  • Provides crisis intervention and mediation as necessary

  • Mentors will maintain a minimum productivity level of 85% billable contact per week

  • Organization and maintenance of client information and case notes

  • Recordkeeping and copying of necessary information, dissemination to appropriate individuals within the agency based on departmental requirements

  • Participate in staff training, supervisory and staff meetings

  • Maintains regular and reliable attendance

Performance/Physical Requirements:

  • Work is performed in the homes of clients and in the community. This may involve extensive driving with own car.

  • English reading and writing skills required

  • Interacts verbally with coworkers, representatives from outside agencies, and families

  • Attention to detail and confidentiality required

  • Works flexible hours

  • Basic computer skills are required

Qualifications:

  • High School Diploma/GED required

  • Must be at least 21 years of age

  • Must have a valid Ohio Driver's License, proof of valid Auto Insurance and a safe driving record

  • Bachelor's Degree in the social sciences preferred

  • Experience working with special needs children is preferred.
