  Enable patients to stay independent while they remain in their own homes. Perform routine supervisory visits to evaluate the plan of care and supervise delivery of excellent client service. Present educational in-service trainings Communicate with Case Managers and other members of the care team REQUIREMENTS Graduation from an accredited school of nursing and current PN or RN licensure.
