Job Description

The Certified Diabetes Educator provides health care under physician supervision. Provides direct diabetes patient education and assists in developing diabetes patient education activities. Assists in the assessment of the program and its goals. Participates in all quality assurance activities; collects, and compiles data for the Diabetes Education Program as well as Medical Home and ACO initiatives. Participates in staff development activities and acts as a liaison with all nursing areas interacting with the program. Complies with WESTMED departmental policies and procedures and maintains good patient and interdisciplinary working relationships

ESSENTIAL FUNCTIONS AND JOB RESPONSIBILITIES:

  • Provides individualized diabetes self-management education/ training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Certified Diabetes Educator

  • Provides direct diabetes patient education and assists in developing diabetes patient education activities.

  • Responsible for utilizing professional skills and experience to educate patients with Type 2 Diabetes and healthcare professionals on the correct administration of an injectable prescription medicines.

  • Acts as a liasion to WMG PCP's Hospitalists, Snifists and and other medical staff regarding newly diagnosed diabetic patients as well as chronic/complex patients to provide/coordinate the medically necessary and appropriate care for the patient.

  • Develops patient care plans in collaboration with patient/caregiver/provider

  • Able to lead and effectively manage individual and group education sessions

  • Be responsible for any ADA or NCQA Recognized Program process.

  • Collects DSME/T program participant assessment data, in a collaborative and ongoing manner.

  • Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSME/T program participants.

  • Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.

  • Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.

  • Conducts a follow-up assessment upon completion of DSME/T program services, using outcome measures.

  • Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records.

  • Utilizes a team approach to provide services and collaborates and communicates with team members when needed.

  • Identifies when a program participant's needs are outside the scope of the instructor's practice and expertise, plus arranges for additional services to meet needs.

  • Acts as a liaison to provide ongoing support and education to the family/caregiver system to lessen enhance self -management and lessen disease anxiety and decrease negative outcomes

  • Solves complex problems and/ or conducts moderately complex analyses impacting disease management, Medical Home and ACO diabetes composite and associated Metrics

  • Develops care plans for complex cases in collaboration with patient/caregiver/provider.

  • Works with minimal guidance, seeks guidance on only the most complex tasks.

  • Translates concepts into practice.

  • Provides explanations and information to others on difficult issues.

  • Coaches, provides feedback, and guides others.

  • Assesses the effectiveness and quality of services provided related to clinical, functional, and financial impact and outcomes

  • Refers appropriately to and maximizes use of community services, support programs, internal/external resources available to patient including Embedded Case Management, Palliative Medicine, Behavioral Health, Diabetic Education, etc

  • Educates and supports patients to assist them in the best decision making for their health and wellness.

  • Act as a resource for others with less experience.

  • Adheres to ethical, legal/regulatory and accreditation standards.

  • Other duties as assigned.

GENERAL QUALIFICATION STANDARDS & JOB REQUIREMENTS:

Education/Certifications/Licenses:

  • Registered Dietician or Registered Nurse, a Master's degree is preferred

  • Minimum of two years clinical experience.

  • This role requires a valid, NY and CT RN license

  • CDE certification through the National Certification Board for Diabetes Educators.

  • Bilingual preferred

Performance Standards for Specialty Knowledge, Skills & Abilities:

  • Insulin pump experience preferred

  • Additional experience in any of the following is desirable: chronic condition management of patients with asthma or hypertension; nutrition teaching, motivational interviewing, weight management

  • Astute triage skills required.

  • Transitional coaching experience a plus.

  • Knowledge of Medicaid, Medicare, and government/CMS regulations preferred.

  • Bilingual preferred.

  • Must have computer skills and technical aptitude.

  • Electronic Medical Record Experience Preferred.

  • Customer service skills

  • Work independently, self-motivated

  • Ability to effectively communicate across all levels of the care team and organization

  • Critical thinking

  • Organizational skills

  • Knowledge of regulatory standards and reimbursement policies appropriate to the position

  • In-depth knowledge about current diabetes treatment management.

Pay Range $72,322 to $95,000

The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience and location of position.
