General Summary or Purpose

This position works with youth in the juvenile justice system and the Juvenile Drug Court Program and is responsible for assisting with drug testing, curfew checks, electronic monitoring and may include mentoring of participants in the Juvenile Drug Court Program.

This position works 10-20 hours per month mainly on evenings and weekends. Hours are flexible.

Unit 2 Juvenile Drug Courts are located in Jamestown, Valley City, and Fargo.

Major Responsibilities & Essential Functions

  • Schedules and meets with the Juvenile Drug Court participants, their families and the juvenile court officer for an intake in order to review the rules and the roles.

  • Conducts and observes alcohol sensor and urinalysis tests on a random basis.

  • Conducts curfew checks by phone or in person when requested by the juvenile court officer or the drug court coordinator.

  • Assists with electronic monitoring and all other monitoring devices.

  • May be required to supervise participants in group outings and special projects.

  • Completes required paperwork.

  • Reports any neglect or abuse as a mandated reporter.

  • Performs other duties of a comparable level or type.

Minimum Qualifications & Certification or Licensing Requirements

Requires a high school diploma and valid driver's license or evidence of equivalent mobility.

  • Employee is exposed to biological hazard in the performance of drug testing.

  • Physicalrequirements can typically be characterized as Light: Work involves exerting up to 20 lbs. Of force occasionally, and/or up to 10 lbs frequently, and/or a negligible amount of force constantly to move objects.

VIEW JOBS 4/3/2021 12:00:00 AM 2021-07-02T00:00 Overview Case Manager Registered Nurse (RN) PRN Days The Case Manager Registered Nurse (RN) is responsible for: * Directly contributing to the creation of an outcomes-based patient care delivery system that is a planned process involving service coordination, monitoring, and management to achieve quality clinical outcomes, patient/physician/payor satisfaction, and cost effective utilization of resources * Maintaining and enhancing payor relationships * Assesses, plans, monitors, and coordinates plan of care from pre-admission to community re-entry through the implementation of case management standards and process * Assesses the individual's personal and medical history, current status, diagnosis, prognosis, and the proposed treatment plan * Proactively communicates with payor source to explore coverage solutions (flex benefits, out of contract, etc.) * Utilizes rehabilitation/LTCH/SNF criteria to promote optimal outcomes for the patient within the boundaries of the diagnosis * Coordinates and facilitates treatment team meetings, plan of treatment and IRF- Patient Assessment Instrument/ MDS, as appropriate * Proactively collaborates with the health care team, payors, community agencies, providers and legal representatives to ensure continuity throughout the continuum * Serves as a liaison with the treatment team and the Primary Care Physician, referring physician, medical director, patient/caregiver(s), and other parties as appropriate Qualifications The Case Manager Registered Nurse (RN) must: * Hold current License as a Registered Nurse as required by the state in which the facility resides * Have 2-3 years of related experience to assess clinical and psychological needs and interface with provisional staff, both external and internal, preferably in a post-acute setting Post Acute Medical, LLC (PAM, LLC) is committed to being the most trusted source for post-acute services in every community it serves by utilizing experienced and dedicated staff to provide high quality patient care and customer service.
