Summary: Responsible for the supervision and safety of individuals.
Multiple shifts available. Full or part time. May require evenings, nights, and/or weekends.
Individual Care: Supervises individual's activities, including completion of daily chores, outings, training, grooming, bathing, toileting, meal-time activities, and provides physical intervention as needed.
Ensures individual safety. Follows a schedule of activities. Assists individuals as needed.
Monitors individual physical and mental health, ensuring that individual needs are met and maintained. May administer individual's medication as prescribed by a physician and accompany to medical appointments when necessary. Reports and documents incidents and/or unusual problems to supervisory or on-call staff.
Models appropriate social skills. Ensures individuals' rights are observed and protected. Completes program chores as assigned; i.e. sweeping, dusting, cooking, shopping, washing and drying individuals' clothing, changing linens, etc.
Assists with menu planning, following menus approved by dietitian, and preparation of meals. Ensures that meals are served family style and that appropriate table manners are modeled by staff during meal time. Ensures compliance with special diets.
Program Services: Assists in implementation of the individual's treatment, habilitation, and/or service plans.
Leads groups as assigned. Implements behavior management procedures as assigned and/or as indicated by a contingency program or the individual's behavior plan. Contacts individual's relatives or other significant others as appropriate.
Completes fire drills, severe weather drills, and nightly bedchecks. Reads individual records and ensures familiarity with each individual's social and medical history.
High school diploma or GED. Valid driver's license and safe driving record. Must meet conditional employment requirements.
