ROLE OVERVIEW and PURPOSE
The Panel Management Coordinator will coordinate panel management, outreach, and engagement efforts related to the residency program guideline requirements. This role will maintain data used to compile reports to ensure compliance with month-end/year-end goals and objectives.
RESPONSIBILITIES
Stays abreast of Accreditation Council for Graduate Medical Education (ACGME) for family medicine residency program requirements related to residents assigned to member panels, including size, age/gender distribution goals, and risk levels
Supports member distribution and assignments to residents to meet goals
Works with Analytics Team to develop reports to support effective panel management and continuity, measure panel performance to ensure goals are met, and
Facilitates transition of member panels with resident graduations and onboarding of new residents, including outreach and communication with members to assist with transition
Conducts outreach activities to help members initially engage or maintain engagement in Neighborhood Healthcare services
Registers members and schedules appointments to support continuity of members with resident physician/resident teams
Communicates appointment information to member/family
Follows up with members to document and ensure members kept their appointments
Documents member medical records, including uploading letters/postcards and transferring efforts made to member account in eClinicalWorks (eCW)
Provides Medicare eligible members information regarding enrollment/benefits and other services available at Neighborhood
Provides outreach and scheduling support to other department/teams, as needed
Tracks, reviews, and processes member actions, such as eligibility, telephone encounters and referrals in the designated logs and/or electronically via eCW
Documents actions taken in member medical records in accordance with current clinic, ACGME, DHCS, and Health Plan regulations/guidelines
Responds to member inquiries and provides enrollments instructions via MyPortal
Attends required meetings, trainings, webinars, and other professional development opportunities
Presents health concepts to individuals and small groups, including providing health center program information
Solicits feedback from members experiences to improve access to health care
Collaborates with manager to develop/enhance outreach strategies for members
Monitors analytical dashboards to maintain department productivity, quality, and compliance standards
Monitors and closes provider panels on a monthly basis, as assigned
Cultivates relationships with referral sources and multi-disciplinary teams, including medical, dental, prenatal providers, case managers, outreach staff, call center staff, and community partners
Distributes members to resident panels according to the ACGME guidelines.
Provides operational administrative and schedule management support for providers to maximize clinic access to residency schedules
Operates to instill confidence in our care and in our facilities to members, fellow employees, and other stakeholders
Impacts member experience by demonstrating courteous and helpful behaviors and a commitment to accuracy
Functions at highest level according to credentials and competencies for the role
Stays informed of organizational activities and promotes the mission and goals
EDUCATION/EXPERIENCE
High school diploma/GED required; bachelor's degree preferred
Two years' healthcare patient services experience required; experience within community healthcare and social service programs preferred
Bilingual (English/Spanish) or (English/Arabic) required
ADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)
Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
Knowledgeable about and experience with keeping details records
Ability to use computers and Microsoft applications, such as Word, Excel, and Outlook
Ability to successfully manage multiple tasks simultaneously
Excellent planning and organizational ability
Ability to work flexible schedule
Ability to work as part of a team as well as independently
Ability to work with highly confidential information in a professional and ethical manner
Physical Requirements
Ability to lift/carry 50 lbs/weight

Pay range: $25.04 - $30.19 per hour, depending on experience.
