The palliative care specialist is a registered nurse who is responsible for providing case management services to assigned populations. He/she utilizes a collaborative interdisciplinary approach to anticipate, integrate and coordinate the patient's plan of care, discharge plan and re-entry into the community

Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.

The palliative care specialist serves as a patient/family advocate and acts as a communication link between the physician, patient, family, health care team, and community resources and payers to increase the quality of care for Palliative care patients.

Qualifications /Training:

  • Basic knowledge of maintaining statistical information.

  • 3 years of work experience in the hospital setting, or related experience.

  • Team coordination or supervisory experience helpful but not required.

  • Requires strong organizational and interpersonal skills and the ability to think critically, problem solve and to work independently.

  • Presentation, training and public speaking skills needed.

  • Excellent written and verbal communication skills are required.

Education/Certifications/ Licenses:

  • Must possess a valid SC license- RN
  • Associates Degree
