The palliative care specialist is a registered nurse who is responsible for providing case management services to assigned populations. He/she utilizes a collaborative interdisciplinary approach to anticipate, integrate and coordinate the patient's plan of care, discharge plan and re-entry into the community
Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
The palliative care specialist serves as a patient/family advocate and acts as a communication link between the physician, patient, family, health care team, and community resources and payers to increase the quality of care for Palliative care patients.
Qualifications /Training:
Basic knowledge of maintaining statistical information.
3 years of work experience in the hospital setting, or related experience.
Team coordination or supervisory experience helpful but not required.
Requires strong organizational and interpersonal skills and the ability to think critically, problem solve and to work independently.
Presentation, training and public speaking skills needed.
Excellent written and verbal communication skills are required.
