SPECIALTY PALLIATIVE PHARMACIST SUPPORTING HOSPICE AND PALLIATIVE CARE PROGRAMS

Identify, develop and monitor quality and affordability statewide clinical pharmacy programs to achieve targets and goals. Create chronic disease state management programs for high cost drugs and high cost/risk populations/conditions.

Create visibility around drug spend/trend and program savings by monitoring, benchmarking and sharing performance of key clinical/operational metrics with leadership and stakeholders. Monitor market trends and pipeline. Develop and implement clinical/operational education and training for staff.

Essential Responsibilities:

Lead statewide identification, development, implementation and monitoring of clinical pharmacy program affordability/quality strategies for prioritized areas. Serve as clinical content and/or program performance owner as specified. Identify and create programs to ensure service targets and goals are achieved.

Create visibility and accountability around drug spend/trend and program savings by developing, monitoring, benchmarking and sharing performance of key clinical program metrics with leadership and key stakeholders for prioritized areas. Adjust strategies as needed to meet program targets/goals.

Lead development of clinical pharmacy staff education for assigned areas. Lead/support training to ensure staff achieve clinical/operational competencies and professional development targets.

Monitor external trends/strategies in managing utilization and/or improving performance for prioritized areas. May include pipeline and clinical/operational forecasting for determining and adjusting financial targets as well as rate setting.

Basic Qualifications:

Experience

Minimum four (4) years of clinical managed care experience (or equivalent) related to utilization management and/or clinical/operational program development.

Education

Doctorate degree in PharmD or equivalent AND completion of PGY1 and PGY2 residencies OR a minimum eight (8) years of experience in a directly related field.

High School Diploma OR General Education Development (GED) is required.

License, Certification, Registration

Current Washington State Pharmacist License by date of hire/transfer.

Current Board Certification (or equivalent) by date of hire/transfer.

Additional Requirements:

Advanced skills in literature evaluation, program development for quality/affordability clinical programs and utilization management.

Advanced proficiency with the Microsoft standard suite including Word, Excel and PowerPoint and Visio.

Excellent verbal (including presentations) and written communication skills.

Project management skills, working with teams, and problem solving skills.

Attention to detail and ability to innovate and apply forward thinking.

Strong ability to understand and work with pharmacy claims data (and medical claims data preferred) to demonstrate clinical value of programs.

Able to multi-task, organize multiple projects, collaborate effectively, lead teams and work with key thought leaders to accomplish desired results.

Preferred Qualifications:

Six (6) years or more years of clinical managed care or clinical health plan experience (or equivalent) related to utilization management and/or clinical/operational program development.

Experience in med management with specialty disease states/medications and high risk populations.

Experience/proficiency in working with productivity data, EMR data, pharmacy and medical claims data.

Experience in developing/leading clinical operational improvements for clinical initiatives or strategies.

Doctorate degree in PharmD or equivalent AND completion of PGY1 and PGY2 residencies (or equivalent) with clinical and managed care training.

The Chaplain is responsible for supportive work providing spiritual and emotional care to patients, families and staff of Swedish Health Services and provides specialist level expertise in assessing and addressing the spiritual and existential needs of patients. The chaplain is also responsible for the provision of supportive services such as debriefings and education for staff. The chaplain is a fully integrated member of the interdisciplinary Palliative Care team and contributes to team goal setting. S/he acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives. This position is the liaison between patients/families and community clergy as needed. The Chaplain supports the needs of the Palliative care team and provides palliative care for the hospital and ambulatory patients as needed. In this position you will: * Demonstrate Swedish core values of respect, justice, compassion, stewardship, and excellence to customers, employees, and visitors; and provides quality service in the performance of work assignments and duties. * Make initial spiritual assessment. Provide spiritual and emotional support to patients/families, staff and volunteers. * Determine and supports spiritual practices in keeping with the patient's and family's religious tradition * Enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops * Identify and interpret cultures and faith traditions that impact health care practice and decisions * Utilize appropriate pastoral care interventions and counseling techniques targeted to intended outcomes Required qualifications for this position include: * Master's Div or equivalent degree * Minimum of 3 years in ministry * Minimum one year in medical setting with palliative care experience, including the care of the imminently dying and their families. * Ordination or commissioning for chaplaincy * Endorsement by ecclesial body for chaplaincy * Current Certification by the Association of Professional Chaplains, National Association of Catholic Chaplains or National Association of Jewish Chaplains; be provisional certified; or be eligible to obtain certification within 1-3 years of employment * Current valid driver's license and functional transportation * Demonstrated PC skills, i.e. proficient with MS Office products including Word, Excel, Power Point and Outlook * Open to and supportive of a wide range of spiritualities * Ability to work with interdisciplinary health care team; knowledge and practice of theology; knowledge and practice of grieving process; knowledge and practice of medical ethical issues * Ability to develop and maintain effective working relationships with peers, physicians, medical providers and other business leaders and departments * Ability to develop and present educational programs for various teams, understanding adult learning principles * Assertive and creative in problem solving * Self-directed and able to work under pressure and continuing change * Flexible and willing to take on increasing responsibility * Ability to work in a high volume environment while maintaining high quality standards * Sensitively manage highly confidential information Preferred qualifications for this position include: * Completion of 4 units of CPE, M.
