SPECIALTY PALLIATIVE PHARMACIST SUPPORTING HOSPICE AND PALLIATIVE CARE PROGRAMS
Identify, develop and monitor quality and affordability statewide clinical pharmacy programs to achieve targets and goals. Create chronic disease state management programs for high cost drugs and high cost/risk populations/conditions.
Create visibility around drug spend/trend and program savings by monitoring, benchmarking and sharing performance of key clinical/operational metrics with leadership and stakeholders. Monitor market trends and pipeline. Develop and implement clinical/operational education and training for staff.
Lead statewide identification, development, implementation and monitoring of clinical pharmacy program affordability/quality strategies for prioritized areas. Serve as clinical content and/or program performance owner as specified. Identify and create programs to ensure service targets and goals are achieved.
Create visibility and accountability around drug spend/trend and program savings by developing, monitoring, benchmarking and sharing performance of key clinical program metrics with leadership and key stakeholders for prioritized areas. Adjust strategies as needed to meet program targets/goals.
Lead development of clinical pharmacy staff education for assigned areas. Lead/support training to ensure staff achieve clinical/operational competencies and professional development targets.
Monitor external trends/strategies in managing utilization and/or improving performance for prioritized areas. May include pipeline and clinical/operational forecasting for determining and adjusting financial targets as well as rate setting.
Minimum four (4) years of clinical managed care experience (or equivalent) related to utilization management and/or clinical/operational program development.
Doctorate degree in PharmD or equivalent AND completion of PGY1 and PGY2 residencies OR a minimum eight (8) years of experience in a directly related field.
High School Diploma OR General Education Development (GED) is required.
License, Certification, Registration
Current Washington State Pharmacist License by date of hire/transfer.
Current Board Certification (or equivalent) by date of hire/transfer.
Advanced skills in literature evaluation, program development for quality/affordability clinical programs and utilization management.
Advanced proficiency with the Microsoft standard suite including Word, Excel and PowerPoint and Visio.
Excellent verbal (including presentations) and written communication skills.
Project management skills, working with teams, and problem solving skills.
Attention to detail and ability to innovate and apply forward thinking.
Strong ability to understand and work with pharmacy claims data (and medical claims data preferred) to demonstrate clinical value of programs.
Able to multi-task, organize multiple projects, collaborate effectively, lead teams and work with key thought leaders to accomplish desired results.
Six (6) years or more years of clinical managed care or clinical health plan experience (or equivalent) related to utilization management and/or clinical/operational program development.
Experience in med management with specialty disease states/medications and high risk populations.
Experience/proficiency in working with productivity data, EMR data, pharmacy and medical claims data.
Experience in developing/leading clinical operational improvements for clinical initiatives or strategies.
Doctorate degree in PharmD or equivalent AND completion of PGY1 and PGY2 residencies (or equivalent) with clinical and managed care training.
