Position Summary:

Reporting to the PACE Director of Nursing, the PACE Clinical Services RN Manager is a proactive manager, responsible for site specific overall quality, compliance, standardization, and performance of San Diego PACE locations, including management of PACE Clinical Services RN Supervisors and other clinical interdisciplinary team supervisors, as assigned. Responsibilities include ensuring regulatory compliance across all PACE locations, hiring and training of Clinic RN's, Day Center LVN's, Clinical Coordinators, and other clinical teams/areas as assigned. This position acts as the management level liaison between the Site Medical Director and Site Manager, to ensure high quality outcomes for PACE participants.

Essential Functions of The Job:

  • Responsible for hiring, training and development of clinical teams throughout San Diego PACE locations Clinics and Day Centers, including but not limited to: proactive trouble shooting, hiring, on boarding new team members, training, staff development, in-services, quality assurance and quality improvement.

  • Creates trainings and in-services in response to staff development needs, or program requirements.

  • Completes annual competencies for all clinical Clinic and Day Center staff and Providers, including direct observation of demonstrated competencies.

  • Ensures all clinical quality measures are achieved, via conduction of routine audits and active participation in PACE QA processes in compliance with PACE requirements and regulations.

  • Assists with the planning and preparation of the PACE annual CMS/DHCS audit.

  • In coordination with the PACE Medical Director and the PACE Quality Manager, PACE Clinical Services RN Supervisor and Site Manager, ensures that clinical protocols and policies are followed, reviewed, and updated as needed; making recommendations for improvements, and instrumental in implementation of QA improvement plans.

  • Ongoing review of policy and procedures, with revisions as appropriate.

  • Monitors and ensures regulatory compliance and standardization of practices across all PACE locations.

  • Conducts routine management meetings, providing updates and best practices to PACE Clinical Services RN Supervisors, PACE Medical Director and Site Manager.

  • Conducts routine developmental 1:1 meeting with each PACE Clinical Services RN Supervisor.

  • Ensures and monitors all clinical related logs (nursing, immunization, refrigerator, etc.) are completed appropriately and timely, medications are not expired and stored in appropriate location and crash carts are complete and up to date.

  • Ensures a safe work environment for all team members, and immediately takes appropriate steps to resolve any risks.

  • Other duties as assigned.

  • Able to work collaboratively with Site Managers, PACE Medical Director, PACE Quality Manager, internal departments, and external service providers.

  • Demonstrated strong communication and customer service skills, problem solving, critical thinking, time management, organizational skills, and clinical judgment abilities.

  • Demonstrated ability to identify best practices across multiple relevant scopes of practice.

  • Creation of engaging, high quality training materials and demonstrated presentation skills.

  • Knowledge of PACE and CMS requirements and regulations.

  • Ability to travel between work sites to transport supplies, as needed.

  • Technical ability to utilize Microsoft Office computer applications, EHR and video conferencing.

  • Complies with all department, organization, and government policies & procedures.

  • Attends meetings and trainings as required.

Job Requirements:

Experience Required:

  • Currently practicing RN
  • 5 years RN experience
  • 1 year experience working with frail elderly populations within a PACE program
  • 3 years' experience in a lead or supervisory nursing role.

Experience Preferred:

  • Knowledge and or experience working in senior or elder care setting - PACE, Nursing Home, Hospice, ADHC.

  • Nursing competency and familiarity with acute care of frail adults, complex wound care and cognitively impaired adults care paths, and instruments commonly used in skilled nursing home settings.

  • Educate and evaluate clinical staff's ability to triage clinical symptoms, applied care paths, clinical interactive tools in patient evaluation and how to effectively communicate with interdisciplinary teams.

  • Must be comfortable assessing clinical ability of staff, including starting IV lines, IV medication administration and monitoring.

Education Required: Graduate of an accredited school of nursing

Education Preferred: Bachelors Level Degree, or greater

Certifications Required:

  • Valid unrestricted CA RN license, valid driver's license and reliable transportation and proof of current vehicle insurance (if applicable),
  • First Aid/BLS certification.

Verbal and Written Skills Required to Perform the Job:

  • Excellent oral, written, and interpersonal communication skills.

  • Bilingual (Spanish/English/Arabic/Tagalog) is a plus, but not required.

Technical Knowledge and Skills Required to perform the Job:

  • Possess the skills to provide high quality care for geriatric patients. Must thrive in team environment, possess good organizational and supervisory skills, and can effectively handle difficult and unusual interpersonal situations.

  • Meet a standardized set of competencies for the specific position description established by San Diego PACE and approved by CMS.

  • Knowledge of video conferencing software and Microsoft Office, including Word, Excel, and Power Point. Knowledge of Electronic Health Records (ie. EPIC, True Chart, NextGen).

Equipment Used:

  • Minor medical equipment such as vital signs monitors, emergency cart, automated external defibrillator (AED), and patient personal assist devices.

  • Networked computer with various applications, copy machines, fax machines, document center, and landline/cell phones. Personal vehicle.

Working Conditions and Physical Requirements:

  • Physical clearance for reaching, bending, stooping, crouching, kneeling, and grasping; have full range of motion; ability to lift 50 lbs; need to lift and participants; TB clearance; must maintain current First Aid, CPR and BLS Certification.

  • Frequent standing and walking; Will be required to occasionally travel to other sites or other locations in San Diego County.

Universal Requirements:
