Position Summary:

The PACE Enrollment Specialist is responsible for the initial screening and coordination of new participants to the San Diego PACE (Program of All Inclusive Care for the Elderly) program. He/she is also responsible for working with the PACE Marketing Manager to implement the program's intake, enrollment, outreach and marketing activities.

Essential Functions of the Job:

  • Conducts San Diego PACE program eligibility screening, intake, education and enrollment assistance activities to initially determine participant needs and eligibility for enrollment.

  • Coordinates with potential participants and/or their families to conduct initial screenings and assessments in the homes of the potential participants.

  • Completes appropriate documentation related to participant assessments as required.

  • Coordinates communications of eligibility decisions, approvals or disapprovals with participant or their family members.

  • Participates in the interdisciplinary team (IDT), management team and other related meetings and activities as needed.

  • Participates in outreach and marketing activities in conjunction with the PACE Marketing Manager.

  • Collects and data enters the daily referral, intake, enrollment, marketing and outreach activities notes with submission of weekly and monthly reports to PACE Marketing Manager.

  • Provide tours of PACE center(s) to interested family members, potential participants, community partners and providers.

  • Facilitates referral from SYHC and community resources for assistance with other needs and complex issues.

  • Provides responsive, high quality service to patients, SYHC employees, representative of County and outside agencies and members of the public by providing accurate, complete and up-to-date information in a courteous, efficient and timely manner.

  • Provides input and feedback on PACE's internal and external outreach, education and enrollment plan for engaging, assisting, enrolling and retaining new and current patients.

  • Conducts public outreach and education activities, individually and in group, to ADHC, SYHC patients and community members at clinic locations and other community locations about PACE and other related health coverage options.

  • Work independently and as effective member of the team.

  • Multi-task in projects and their respective activities, timelines and issues.

  • Able to adjust outreach and enrollment strategies as necessary based on data analysis.

  • Establish and maintain effective interpersonal relationships with all levels of staff, other departments, agencies, providers and the general public.

  • Complies with all department, organization and government policies & procedures.

  • Attends meetings and trainings as required.

  • Adheres to and models SYHC's core values and behaviors of Excellence, Empowerment, Integrity and Respect.

  • Adheres to SYHC and PACE attendance and punctuality policies and practices.

  • Performs other related duties as assigned or requested.

Job Qualifications:

Education required: High school diploma or GED equivalent.

Education Preferred: Bachelor's Degree in a health or human service field.

Experience: A minimum of one year working in community health, senior health, marketing, or enrollment for a public health, social service organization or health plan. One year of MediCal, Medicare, managed health care eligibility or senior related sales. Working with the frail or elderly population is a plus.

Certifications/Licenses: CA driver's license with appropriate automobile insurance required. Must take and pass the State of California DHCS PACE and Medi-Cal Marketing Certification exam within 60 days of hire.

Verbal and Written Skills to Perform the Job: Must be able to communicate effectively and comfortably in public forums. Prepare concise and clear reports on marketing and outreach. Bilingual (English/Spanish) is required.

Technical Knowledge and Skills to Perform the Job: Good knowledge of Microsoft Word applications including Excel, Access and Power Point. Computer data entry, analysis and reporting experience is required. Knowledge of electronic health data systems including NextGen is preferred.
