Under the direct supervision of the Access to Care Supervisor, the Access to Care specialist is responsible for monitoring all activities related to companionship arrangements and for recording all dialysis schedules on SD PACE Electronic Health Record.
Essential Functions of the Job:
Responsible for ensuring all companionship needs for participants are address appropriately.
Provide guidance, for all staff attending companionship
Main point of contact for inquiries from staff while out on companionship
Create companionship list for PACE locations
Perform reminder calls to participants to confirm the need for companionship 48 hours prior to appointment date.
Perform reminder calls to Specialist offices to confirm all appointments that require companionship 48 hours prior to appointment date.
Send confirmed companionship list 24 hours in advance to floor coordinator for CNA/PCA assignment
In Collaboration with Floor Coordinator to develop an accurate companionship list on a daily basis
Record on SD PACE Electronic Health Record calendar the dialysis recurrence for all participants
Maintain an accurate list of all participants receiving dialysis services for both locations
Coordinate transportation for all dialysis trips with the Transportation vendor
Works closely with other departments to ensure efficiency in processes
Prepares reports requested by supervisor where information must be obtained from a variety of sources.
Additional Duties and Responsibilities
Communicates effectively with participants and co-workers
Performs other duties as assigned.
Works effectively in a team setting.
Adheres to SYHC and PACE attendance and punctuality policies and practices.
Maintains all records and documentation as required.
Education Required: High school diploma or GED equivalent required. Post High School Vocational or Specialized Training preferred
CA valid driver's license and reliable transportation
Proof of current vehicle insurance (if applicable)
Required -At least one year experience working with the frail elderly population
One year caring for seniors or elderly in an assisted living program, nursing home that provides services to disabled or elderly adults.
Technical Knowledge and Skills Required to Perform the Job: Must possess the skills necessary to provide care for geriatric patients, must thrive in team environment, must possess good organizational and supervisory skills, ability to effectively handle difficult and unusual interpersonal situations. Meet a standardized set of competencies for the specific position description established by SYHC PACE and approved by CMS.
