Job Summary:

The City of Greeley is seeking a dedicated Outreach supervisor, who is passionate about Homeless Solutions and Community Outreach Resources. The Outreach Supervisor will provide direct supervision to the outreach team and ensure that this role will be responsible for coordinating and overseeing daily activities of Outreach Case Managers and providing oversite of the utilization of outreach services and resources within the program. This individual will be a working supervisor and will support the teams efforts to engage homeless via street outreach activities, coordinate referrals and program-linkage to additional programs serving homeless, maintain thorough and accurate records in both written and electronic form, and collaborate with other homeless outreach services within the community. The team supervisor will ensure programming guidelines are followed, proper documentation is being kept and assist with training in the areas of HMIS.

Experience, Knowledge, Skills:

Minimum Requirements

  • Bachelor's degree in behavioral health field (Sociology, Psychology, Social Work, Criminal Justice, etc) or unrelated field with one year of documented work in behavioral/social services.
  • Bachelor's degree in support services or related field of study.
  • Two (2) years of mental/behavioral health direct care experience.
  • Two (2) years of demonstrated experience in managing Housing First Intensive Case management.
  • Two (2) Extensive experience working with and servicing people with very complex needs, including but not limited to chronic homelessness, serve trauma, substance use disorders, mental illness.
  • Two (2) years of supervisory experience.

OR

  • Any combination of related education, experience, certifications and licenses that will result in a candidate successfully performing the essential functions of the job.

Preferred

  • Bilingual in English and Spanish.
  • One (1) year Housing first services experience.
  • Master's degree in support services or related field of study.
  • Exceptional skills in Microsoft Word, Excel, Access, PowerPoint, or other equivalent software packages.
  • Six (6)+ months of experience working within a municipality.
  • Three (3) years' experience in managing Housing First Intensive Case management.
  • Three (3) years of supervisory experience.

Essential Functions:

  • Oversees daily activities of Outreach Case Managers and provides oversite of the utilization of outreach services and resources within the program.
  • Completes coordinated intakes, assessments, and screenings to determine housing eligibility based on HUD and Colorado State Department of Housing (DOH) guidelines
  • Very familiar with HMIS and understands the need to have strong data to tell our stories. Assist case managers with proper documentation in HMIS.
  • Ensure best practices, such as Housing First, Harm Reduction, and Trauma Informed Care etc., are utilized and incorporates at all levels of the team's functioning.
  • Engages homeless via street outreach activities, coordinates referrals and program-linkage to additional programs serving homeless, maintains thorough and accurate records in both written and electronic form.
  • Conducts triage to determine and assist with immediate client needs including food, water, and emergency shelter
  • Oversees the scheduling for the Outreach team and always ensures coverage. Will serve as back up when short staffed and be on call as needed.
  • Leads daily huddles for the Outreach team
  • Ensures coordination with the HMIS system and individuals are placed on the housing priority list
  • Assists in program development and quality assurance for grant activities as directed.
  • Supervisors Case managers with helping connect individuals to resources, placing them on the housing priority list by completing the vulnerability assessment, ensuring individuals are doc ready, and connecting to housing resources, and overseeing bridge housing till a housing resource is available
  • Attends Homeless Solutions trainings and workshops related to housing
  • Attends monthly case management meetings, meetings with supervisors and staff meetings as scheduled
  • Cultivates new and existing partnerships in Greeley and the surrounding area to connect individuals to housing
  • Responsible for reading all logs/emails in a timely manner to ensure you are up to date with current information pertaining to the residents
  • Performs other duties, as assigned

Supervisory Responsibilities:

  • Coordinates and oversees daily activities of Outreach Case Managers and provides oversite of the utilization of outreach services and resources within the program.

  • Ensures compliance with safety procedures.

Travel:

  • This position could travel up to 10%

Work Environment:

  • Work is exposed to hazards that are typically found in outdoor environments.
  • Exposure to all weather conditions.
  • Subject to driving City vehicles in all weather conditions.
  • Extensive computer work primarily performed at a desk.
  • Employees are subject to working alone, with groups, and/or attends meetings.
  • May be required to conduct business in alternate locations within City buildings or field sites, sometimes in formal setting
  • May be required to occasionally work extended hours in the evening, holidays, or weekends.
  • Occasional interruptions and be required to manage competing priorities with inflexible deadlines.
  • Frequent contact occurs with employees and the public, in person and on the telephone.
  • Occasionally required to sit and talk or hear customers.
  • Communication skills to interface with other departments and agencies.
  • Operate standard office equipment requiring continuous or repetitive hand/arm movements.

Physical Requirements:

  • Vision enough to interpret computer screens and documents to assist the public and complete work assignments; specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
  • Use of hands to finger, handle, or feel and reach with hands and arms.
  • Flexibility to bend, twist, and reach out to load and unload supplies.
  • The essential functions of this job require frequent hearing and repetitive motion; continuously sitting and talking; and regular carrying/lifting and moving objects up to 50 lbs.
  • Ability to move over rough, uneven, and rocky surfaces
