# # # # Role may work any of 7 days a week. # # # # ##Role will include outreach to patients with pain to ensure FU care after being seen in any of the 3 BHS EDs. DEFINITION/PRIMARY FUNCTION # In response to a provider order or high-risk patient identification, the ALERT Care Navigator is responsible for coordination of wrap around services for patients with pain and/or substance use disorders seen in the Emergency Department using a trauma informed lens. In conjunction with partner agencies, the goal is to optimize the physical and emotional health of this diverse population of patients. Additionally, the ALERT Care Navigator is responsible for implementation and management of the comprehensive program of screenings, referrals and providing printed educational materials as recommended by the Emergency Department provider. Engaging the patient, increasing access and timeliness of care is critical to high quality care. The Navigator works closely with ED providers, leadership and staff, specialty providers and staff, SUD treatment providers and programs, and broad community agencies across the continuum to deliver these services. This position requires participation in telehealth visits and in-person visits to continually strengthen the relationship with the patient and family support system as well as availability to see patients in the ED while the ALERT Care Navigator is in the department. POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.) Experience: Two years of experience in a clinical setting. Prior experience working with individuals to assess their needs and assist in goal setting is preferred. Lived experience chronic pain or substance use disorder is preferred. Education and Training: High School diploma is required, and an Associate#s degree, nursing education or Community Health Worker designation is preferred. Must complete all mandatory trainings required by the License, Certification

Other Requirements: Familiarity of harm reduction, strength based motivational interviewing, trauma informed care # Comfort administering pain, depression, social determinants of health and substance use screening tools Proven ability to effectively collaborate with individuals/agencies, optimize outreach activities, and participate in program planning # implementation. Experience with tracking, trending # analyzing data to create the action plans for performance improvement. Working knowledge of community and health system resources Excellent interpersonal skills with a variety of people including patients, families, staff, physicians, and agencies required. Proven ability to exercise independent judgment and initiative.
