Overview

Required Skills and Experience

- Looking for Retail, hospitality background. Outbound cold calling in a call center, Sales

roles need to be able to talk to anyone.

-NOT healthcare, not care coordinator, or case mgt.

- MUST have passion for home health care and community health

- Screen for, " are they comfortable with working in a metrics driven environment?"

- Massachusetts Unrestricted Driver’s License and Car

- Priority given to Spanish Bilingual candidates.

What you need to know:

• Work a full-time 40-hour week, Monday-Friday 9am to 5pm ET and participate in

a Saturday coverage rotation

• You have a passion for working within the community you are a part of, or have

been a part of in the past

Additional Job Details:

- number of positions: 4

- not clinical, more of a sales role. Will be provided with a call list

- NO Case manager or Care Coordination profiles

- Will enroll members through outreach via phone, electronic and traveling to

individual's homes

- Pay Range: $17-$23/hour depending on experience

- Massachusetts Unrestricted Driver’s License and Car

- Target Start Date: 8/24

- Work a full-time 40-hour week, Monday-Friday 9am to 5pm ET and participate in a

Saturday coverage rotation

About the Role:

• You will work in a radically different model of healthcare

• Work alongside a care team who will be responsible for each member's ongoing

care.

• Responsible for creatively locating and connecting with members that we believe

will benefit from the Cityblock care model

• Be a member’s point person during the initial connection to Cityblock, available and

accessible to assist with various social needs, to troubleshoot, to educate, and to

be a listening ear.

• Provide telephonic, written, digital and face to face outreach and engagement

services for each eligible member. This also includes member events, group

welcome events and community outreach events.

• Leverage available data from Commons to learn more about potential members and

inform your outreach process strategy.

• Complete the intake and consent process for eligible members, and handoff to the

Community Health Partner upon full enrollment

• Engage and foster trust quickly with members, acting as the face of Cityblock and

the key point-of-contact to assist members prior to full enrollment

• Meet with members in their homes, neighborhoods, and the Cityblock health hub;

nearly all your work is done in the field

• Participate in case conferences for members you have engaged to recommend the

appropriate team and interventions

• Work daily within Commons to collect data, organize information, track tasks, and

communicate with staff, patients, and family members

• Provide feedback to the product development team, and, over time, becoming part

of a super-user group to assist in onboarding and supporting others
