Under the general direction of the Psychosocial Team Leader and Care Coordination manager, serves as a resource to HIV patients, family and friends, providing information, referrals and support as well as providing counseling and testing services. This position specifically supports the HIV Care Coordination project that provides enhanced interagency interdisciplinary services to patients who have not succeeded with standard Ryan White support services.

DUTIES AND RESPONSIBILITIES:
1.Identify patients suitable for enrollment into the Care Coordination project and, with the care coordination nurse and staff from partner agencies, initiate enrollment into the program. This may occur in the ID clinic, in the DMC hospitals, and elsewhere.
2.Help coordinate and participate in interdisciplinary HIV care teams both within the Tolan Park ID clinic and with the participating Care Coordination agencies.
3.Provide Pre and Post HIV test counseling, point of service HIV testing, refer as appropriate and maintain records for MDHHS.
4.Provide Health Education and Risk Reduction strategies to patients as necessary.
5.Provide access to Partner Notification services by referring information to the Detroit-based Disease Intervention Specialists.
6.With the WSU Care Coordination nurse, coordinate care between different medical, behavioral health, and social service providers.
7.Assist patients to understand the process of seeing physical health and behavioral health care providers including other specialists for their health care needs, and assist patient in getting Tolan Park and other appointments and referrals as necessary.
8.Assist patients to understand community programs, health and prescription insurance.
9.Provide emotional support to patients and their support systems.
10.Document services in the medical record and in CareWare.
11.Other duties as assigned

?Bachelor's degree.

Preferred Qualifications

Experience working with the HIV community preferred

?Interpersonal skills to deal with families of patients, patient and agencies involved in providing services

?HIV Counseling and Testing certification preferred

?Basic knowledge of e-mail and Microsoft Office application software.
