We operate an ambulatory medical clinic located on the Hoopa Valley Indian Reservation, partially funded by the Indian Health Service. The clinic is solely owned and operated by the Hoopa Valley Tribe. We are a Joint Commission accredited ambulatory clinic and behavioral health unit (most recently passed the survey in 2021), with our Patient Centered Medical Home accreditation.

Our primary programs and services consist of outpatient medical, dental, pharmacy, outreach services, senior nutrition, ambulance service, a grant funded diabetes department, and behavioral health unit (which includes mental health and substance abuse). Hoopa is located on the beautiful Trinity River in the Northern California coastal mountains. Hoopa is a small community of approximately 4,500 residents: primarily Native American. Our clinic services are open to the public.

The position is a full-time, regular staff position that includes benefits such as: a 401(k) Safe Harbor Plan (100% vested, KMC up to 4% match); 13 paid holidays; medical, dental, vision, and life insurance; vacation leave of 8 hours per month, increases based on years of service; and sick leave of 8 hours per month (static). Hours of work are Monday through Friday, 8AM to 5PM. Work weeks are based on 80-hour pay periods, and employees are paid every other Friday.

Duties:Functions and Responsibilities • Knowledge of basic supervisory principles and practices. Participates as a team leader on a health team with other professionals and paraprofessionals in the delivery of public health/nursing services. • Assigns client cases and assists in evaluation of staff. • Provides consultation and guidance to team members on difficult client problems; assists in the training and orientation of staff.

Collaborate with Providers and RN Care Managers to assist with provider driven continuity of care. • Evaluates the health needs of individuals and special population groups from a wide variety of cultural and economic backgrounds. • Identifies the symptoms of physical, mental, or emotional problems and refers individuals and families to appropriate services medical, mental health, or other support services; performs follow up on such referrals. • Coordinates and collaborates with Tribal Public Health, County Public Health, and State Public Health. Establishes and maintains effective working relationships with clients, public and private social and health agencies, physicians, and the public. • Gives immunizations and conducts immunization clinics; may perform selected diagnostic tests and gives prescribed treatments according to provider driven orders or approved protocols. • Provides prenatal and post-partum nursing care and participates in well-child clinics. Knowledge of maternal, child and adolescent health principles and practices. • May serve as primary staff in communicable disease clinics or in community outreach. • May conduct a specialized health care or training program or project; assists in planning and development of programs and preparation of funding proposals. • Manage, monitor, and evaluates grants, programs, projects, and special services; prepares a variety of periodic and special reports for appropriate management review.

Must be able to prepare clear, complete, and accurate documentation, reports, and other written materials. • Manage a complex caseload and establish priorities for case management, treatment, and referrals. Carries out provider ordered treatments within the home setting, including, wound care, dressing changes, urinary catheters changes, etc. Following up on recommended treatment plans to ensure effective resolution of health and related problems • Knowledge of principles, practices and current trends of public health nursing and the California Nurse Practice Act rules and regulations and applicable state and local codes. • Knowledge of demographic, biostatistical and data collection methods utilized in evaluating public health needs and problems.

Identifying community health needs through analysis of demographic and biostatistical data and information. • Other duties as assigned Skill in: • Planning, assigning, directing, and reviewing the work of others. • Managing a complex caseload and establishing priorities for case management, treatment, and referrals. • Applying the principles of epidemiology to a wide range of social and health problems. • Performing nursing assessments and selected diagnostic tests and giving prescribed treatments. • Identifying community health needs through analysis of demographic and biostatistical data and information. • Exercising sound independent judgment within established guidelines. • Assessing normal and abnormal health and behavior, finding, and developing effective treatment and referral plans. • Communicating clear and accurate information regarding clients to physicians, health providers and other health members. • Following up on recommended treatment plans to ensure effective resolution of health and related problems. • Preparing clear, complete, and accurate documentation, reports, and other written materials. • Providing group and individual health care instruction. • Assisting in the evaluation of Community Health Outreach Program effectiveness. • Establishing and maintaining effective working relationships with clients, public and private social and health agencies, physicians, and the public. • Proficient in treatment and procedures carried out in the ambulatory care setting and home setting, wound care, urinary catheter, dressing changes etc. SUPERVISORY RESPONSIBILITIES: Directly supervises employees within the Community Health Outreach Department.

Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include participating in the Human Resource retention and recruitment operations, including interview, and hiring process, orientation/training of employees, and employee evaluation. A primary function in the supervisory role is creating a team environment, promoting a growth mindset, fostering, and enhancing positive communication skills within the team.

Qualifications:QUALIFICATIONS MINIMUM MANDATORY QUALIFICATIONS: Current License as a Registered Nurse by a Board of Nursing.

Must maintain continuing education for nursing licensure current. Certification: CPR and/or ACLS certification. (Preferred) Education: Bachelor's Degree in Nursing and Public Health Nursing Certificate from an accredited college or university.
