Short Description


The Outreach Healthcare Assistant works directly with the Coalition's Care Management Project, an initiative to conduct outreach and transitional primary care for high utilizers of Camden emergency departments and hospitals.

The Outreach Healthcare Assistant will work together with a team of a social

worker and a nurse practitioner, conducting home visits and outreach to enrolled patients.

This position reports directly to the Care Management Project's Nurse Practitioner.

Special Requirements

Scheduled Days / Hours: Monday

  • Friday 8:30am

  • 5:00p;


High School Diploma or Equivalent; medical assistant training program certification required.


Fluent in English and Spanish.
2-3 years experience working in a healthcare office setting (experience working in a Camden medical office preferred).

Competent in basic computer skills; knowledge of using an electronic health records system required.

Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.

Ability to take direction, follows through with tasks, and work independently.

Flexibility, optimism, and a cooperative working style.

Familiarity of Camden and service agencies/organizations serving Camden residents a plus.

This position requires significant time spent in the field throughout the City of Camden.

Self-provided transportation is preferred.

Work closely with medical providers, primary care teams, health coaches, and other agencies to improve patient care and outcomes. Provide support to patients and their families to help teach and train in areas of health literacy, health behavior modification, social skills and other health and life skills needed to manage health conditions. Work in various settings, including the hospital, primary care and specialty care practices, patient homes, and community settings. Job Duties: * dentify unique needs of patients and directs them to appropriate patient support services (sources for emotional, financial, administrative or cultural support). * Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff * Help patients develop health management plans and goals * Follow-up with health management/care plans with both patients and providers * Coach patients in effective management of their chronic health conditions and self-care * Assist patient in understanding care plans and instructions * Work collaboratively and effectively within a team * Establish positive, supportive relationships with participants and provide feedback * Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible * Assist clients in accessing health related services * Continuously expand knowledge and understanding of community resources * Facilitate communication and coordinate services between providers * Motivate patients to be active, engaged participants in their health * Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions Special Requirements Scheduled Days / Hours: M-F; 8a-4:30p Education: Completion of Medical Assistant training program; some college courses preferred Experience: At least 2 years as medical assistant in family practice. Home visiting experience preferred Required License/Certifications/ Registration: NJ certification as medical assistant . Bilingual (Spanish) First Aid certification up to date (including pediatric) Other Qualifications: Verifiable good driving record and reliable transportation with valid insurance. Background check and fingerprinting. Strong interest in family and pediatric healthcare. Familiar with Camden city and Camden area resources required.
