Short Description


The Outreach Healthcare Assistant works directly with the Coalition's Care Management Project, an initiative to conduct outreach and transitional primary care for high utilizers of Camden emergency departments and hospitals.

The Outreach Healthcare Assistant will work together with a team of a social

worker and a nurse practitioner, conducting home visits and outreach to enrolled patients.

This position reports directly to the Care Management Project's Nurse Practitioner.

Special Requirements

Scheduled Days / Hours: Monday

  • Friday 8:30am

  • 5:00p;


High School Diploma or Equivalent; medical assistant training program certification required.


Fluent in English and Spanish.
2-3 years experience working in a healthcare office setting (experience working in a Camden medical office preferred).

Competent in basic computer skills; knowledge of using an electronic health records system required.

Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.

Ability to take direction, follows through with tasks, and work independently.

Flexibility, optimism, and a cooperative working style.

Familiarity of Camden and service agencies/organizations serving Camden residents a plus.

This position requires significant time spent in the field throughout the City of Camden.

Self-provided transportation is preferred.

Behavioral Assistants (BAs) work with youth and families, assisting them with addressing topics such as: behavioral challenges, anger management, low self-esteem, lack of confidence, increasing socialization, communication, interpersonal relationships and familial issues. BAs must demonstrate the ability to work collaboratively with other providers on the youth's team, as well as be an active team member. The BA must possess excellent interpersonal skills and the ability to work with people in varied positions/agencies, and with children of all levels of functionality. Additionally, the BA must be willing to accept varied job assignments, including on-site and off-site training, toileting, feeding, etc. A Bachelor's Degree is required to obtain certification as a BA in the State of NJ. The BA will: * provide direct service to youth in the home and community of the youth. * implement behavioral interventions * carry out BAISP plans developed by the clinician assigned to the case. * provide authorized service hours as determined by the referring agency. * maintain contact with the referring agency. * submit all required paperwork in a timely manner. * attend required supervision to comply with Medicaid standards. * attend Child/Family meetings as required by the referring agency. * attend staff meetings and training as required. * obtain and/or maintain BA Certification (must be obtained within 6 months of hire). * demonstrate proficiency in the use of the Electronic Health Record.
