Job Title: Outreach Coordinator
Roles & Responsibilities:
The Outreach coordinator's primary focus is to ensure that our community of members complete their preventive care exams and assist in scheduling those exams. Outreach will handle all outbound communications to our members via phone calls and mail correspondence including but not limited to Plan updates, Quality incentive letters, Birthday cards, etc. Outreach team will also be back-up to the MSR department and offer coverage/assistance whenever necessary.
Handles incoming telephone calls and answers questions for members and providers, and assists members in accessing all necessary covered and non-covered services in conjunction with plan of care.
Contacts members and engages in QA conversations/questions
Inquires if members had Preventive care exams.
Contacts providers to confirm appointments.
Faxes attestation /checklist to provider.
Updates member schedule status and documents in spreadsheet
Documents in eCare all scheduled appointments.
Checks eCare for Attestations received and update spreadsheet
Responsible for member outreach communications i.e. Quality incentive letters, monthly Birthday Card, I'm Sorry Cards.
Assists members in arranging appointments with physicians and other health care provider.
Coordinates and schedules pick up /drop off transportation to and from scheduled appointments.
Facilitates communication between members, subcontractors, providers and Care Managers.
Communicates effectively with members with special needs by utilizing mechanisms such as: Interpretation vendor, TTY (Tele braille, large prints Braille) talking adaptive equipment, etc.
Assists members with accessing translation services.
Provides plan information to new members, family members or other representatives.
Participates in special programs in response to program needs
High School Diploma
Minimum 3 years job-related experience in healthcare/medical setting
Bi-lingual (English & Spanish, Russian or Cantonese/Mandarin)
Excellent communication skills and phone customer service
Basic computer literacy skills
