Job Description:

Develop and implement Outreach Program components in the chapter to include but not limited to mentoring and T1D Connections Program, Bag of Hope distribution and reporting, outreach event (support or education) planning and coordination, and National handoff follow-up.

Help to expand JDRF's presence in the local communities that the chapter serves; network with hospitals, healthcare providers, and community partners, such as camps and school nurse organizations, to market the JDRF Outreach Program.

Work with chapter Associate Executive Director to identify, solicit, recruit and develop volunteers for leadership and support roles in implementing JDRF Outreach Programs.

Work within chapter budget to ensure that the chapter is fiscally responsible, and additionally assists with local grant resources to fund Outreach projects.

Assist in the identification, management, coaching, and stewardship of new family Walk teams.

Manage and oversee accurate recording of information in database, file system, and/or records.

Work in conjunction with other development staff as needed to ensure Outreach presence and impact at all chapter events; assist with other chapter activities as needed.

Be familiar with all JDRF internal Outreach and related resources and publications; maintain a good understanding of JDRF's mission and research efforts.

Create a bank of personal stories from JDRF families to have available for open and/or unplanned media opportunities.

Respond to and follow through with inquiries, needs, complaints and/or issues in a prompt and courteous manner.

Who You'll Be Working With: As a member of the Clinical Analytics and Innovation team you will work with internal and external Clinicians, Analysts and Provider officers, pharmacists, health economists, and program coordinators, you will have the opportunity to make a profound impact on the lives of people with various types of insurance who have serious medical conditions. You will serve as a critical link to connect patients with case management services. What You'll Be Doing: * Make telephonic outreach to the "yet-to-be-reached" and "lost-to-contact" patients. * Utilize a variety of outreach strategies to engage various populations. * Identify the gaps in use of services such as physician access through discussion with patients. * Educate and inform patients on the services/programs offered. * Identify patients' willingness to access and enroll in the program. * Identify and address the barriers associated with accessing the program. * Utilize engagement strategies to build rapport with field nursing staff * Works closely with the clinical team to support patient engagement and confirm the patient has connected with resources, as appropriate. * Contact with provider offices and other clinical settings to obtain clinical data and/or patient demographics to facilitate care. * Communicate with nursing staff for situations that require nursing judgment. The Experience You Need (Required): * 3-5 years of experience in healthcare, preferably in a setting with patient contact and outreach efforts, service-oriented. Finishing Touches (Preferred): * Bachelor's degree. * Bilingual English and Spanish. * Must demonstrate excellent interpersonal communication skills, including exceptional listening skills. * Must have the ability to positively influence others with respect and compassion. * Must demonstrate intellectual independence and willingness to learn new skills. * Must be flexible and comfortable with ambiguity and change. * Experience working directly with patients, physicians, physician office staff, healthcare team, etc. telephonically (i.e. call center). * Experience with coordinating care, advocating, or navigating on behalf of patients. * Experience with Medicare, Medicaid, third-party insurance, copayments, etc. * Knowledge of telephonic translator services and how to use them. * Strong work ethic built on a foundation of proactivity and teamwork. * Ability to navigate ambiguity with the aid of structured problem-solving techniques. * Computer proficiency in Microsoft Office. * We are looking for team members who possess enthusiasm, a strong work ethic, integrity, who are kind and empathetic, have a sense of humor, and who enjoy working for a collaborative, team-oriented system.
