Outreach Coordinator Position Summary:

Under the direct supervision of the Administrator, this position will promote and market St. Clair Nursing Center. This will be done utilizing both the clinical and business aspects of the position. This will be accomplished by formalizing partnerships with Acute Care providers and local referral sources. This position will have a current RN or LPN licenses. Have a flexible schedule to participate in scheduled and scheduling of meetings to best accommodate those we are trying to serve. Post-Acute Care Coordinator will maintain knowledge of St. Clair Nursing Center and Americare policies and Procedures. Capable of communicating, meeting with and providing information to our Regional Operations Director, Regional Nurse Consultant, Corporate managed Care coordinator and to the Facility Administrator. Must have a knowledge of marketing, business skills, professional appearance and demeanor. Substantial travel is involved with this position also.

Essential Functions:

  • Responsible for maintaining existing and growing new partnerships with Acute care hospitals.

  • Responsible for maintaining existing and growing new partnerships with local referral sources. ( hospitals, Physicians, Clinicians and ALF's)

  • Grow our Long Term and Rehab census

  • Responsible for scheduling, coordinating and presenting at C-Suite meetings

  • Responsible for calling on and building our relationships with the St. Louis Medical market

  • Work with our Americare ALF's to build on our continuity of care.

  • Responsible for making contact with potential residents while they are in the hospital

  • Responsible for initiating advanced care planning with new admissions

  • Tracks all of our referrals and reports them to the Administrator and Regional Operations Director on a monthly basis

  • Will participate in local marketing of the medical and business community and participate in the local wagon wheel activities of the community

  • Follows up with our in-house rehab residents on at least a weekly basis. Follows them once rehabbed to home on a weekly basis for one month

  • Attends weekly Medicare meetings at St. Clair Nursing Center

  • Attends the monthly Barnes PAC meeting .

  • Attends The Local Chamber sponsored events and meetings

  • Will attend at least 1-2 daily stand up meetings at St. Clair weekly

  • Attends/Assists as needed with Quarterly Hometown Hospitality Events

  • Monitors our readmission rates, reports and follows up on them with the administrator, DON and MDS coordinator

Work Environment

This job operates in a professional office or home office environment depending on location as well as outdoor exposure when generating community leads. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Operate a car for required travel.

Position Type/Expected Hours of Work

This is a full-time position. Days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m. CST with hours varying depending on events and meeting schedules.

Qualifications/ Education/Experience/

  • Minimal 18 years of age with high school diploma or GED

  • Prefer college classes in sociology and or psychology or business.

  • Ability to read and write well and document appropriate findings and assessments.

  • Excellent communication and personal interaction skills. Must interact well with public.

  • Knowledge of marketing, business skills, professional appearance and demeanor.

The Opportunity Our Lifestyle Coordinator plans, organizes, facilitates and directs recreational activity programs for residents in the nursing facility. Prepare for events by obtaining and organizing necessary materials for activities. Directs and organizes such activities as sports, dramatics, games and arts and crafts to assist residents to develop interpersonal relationships, to socialize effectively, and to develop confidence needed to participate in group activities. Regulates content of program in accordance with individualized patient capabilities, needs and interests. Organizes and coordinates special outings and may accompany residents on outings. Plan and document a monthly activity calendar. Participate in assessing and documenting progress charts and periodic reports for medical staff and other members of the treatment team reflecting resident reaction and evidence of progress or regression. Network with community organizations, businesses, schools and individuals to form a solid volunteer program. Participate in planning and implementing internal and external community events.
