Primary Duties and Responsibilities

  • Will identify, locate, and engage homeless individuals which meet the homelessness definition and link them to needed to services with in local mainstream community resources.

  • Under supervision of the Licensed Program Supervisor, the Outreach Case Management Specialist will identify homeless individuals within the local three county area and engage them for services with the ultimate goal for them to agree and obtain permanent housing.

  • Upon identification of needed resources, the Specialist will provide community based referrals which may include linking to needed services such as housing, medical, legal, and or other sources.

  • Will record individual case needs information, update case files, and provide case status information into the HMIS system in order to provide a complete record of enrollment services so as to meet requirements of this grant.

  • Will assure that individuals are linked into interim services if substance abuse or mental health services are unavailable at that time.

  • Collects, maintains, and organizes data related to specific quarterly and annual reporting requirements of the PATH grant and will train Outpatient sites so that the homeless individuals can obtain needed services.

  • Facilitates persons served movement through the community service delivery system, tracks and documents persons served progress, and maintains a written record of said activities.

  • Required to meet and maintain TCHS Approved Driver Qualifications per policy number 100.007.52 and transport TCHS person served in any agency vehicle.

  • Performs other reasonable and related duties as assigned.

Minimum Training and Experience

Requires a high school diploma or GED. Two (2) years of full-time experience in a mental health or addictions treatment setting or experience with homeless individuals is preferred. A Florida Certification Board certified recovery specialist may substitute for required years of experience.

Interviews and orients clients and collects information necessary to complete the intake process, including employability recommendations. 2. Under the supervision of the direct supervisor, the employee records individual case information, updates case files, and provides case status information in order to provide a complete record of services. 3. Confers with supervisor and other team members at staff meetings, providing and/or receiving information as necessary to assure proper case management. 4. Collects, maintains, and documents data related to specific reporting requirements of the assigned program, other special client programs, and intra-agency referrals, as assigned by the supervisor. 5. Provides specific community liaison services as assigned by supervisor. 6. Provides the person served with orientation to the agency. 7. Teaches interactive alcohol/drug education classes and provides other educational services when assigned. 8. Completes urine specimen collection and documentation per agency collection procedures. 9. Provides the person served with individual and /or group counseling to facilitate case management goals/objectives 10. Maintains a caseload of up to 15 adults. 11. Assess the person served needs and will work with them to develop natural support systems and linkages. Seek and encourage the involvement of relatives, friends and significant others, peers and volunteers to increase the social support network of the person served. 12. Refer and link the individual to behavior health services and supports to help prevent re-offense, re-arrests, decomposition, and hospitalization/re-hospitalization. 13. Respond to crises and provide intervention strategies with providers to minimize hospitalization or incarceration. 14. Assists with the access of resources and ensure each person served is able to make use of the resources. 15. Advocate on the persons served behalf. 16. Monitor delivery of services. Reassess the need for adjustment to the plan. 17. Help the person served set goals and ensure goals set by providers of services tie together. 18. Discharge planning and follow up. 19. Educate and empower the person served to their full potential. 20. Performs other reasonable and related duties as assigned. Minimum Training and Experience Required a Bachelors degree or three (3) years experience in the Human Service, Behavioral Health or Criminology experience. Must obtain forensic evaluation training within six (6) months of hire. Must obtain twenty (20) hours of training annually to include all agency-required trainings.
