Duties and Responsibilities:

The individual hired will facilitate groups and provide individual

counseling, treatment planning and case management in both the IOP, OP and PHP services.

Review pre-admission data, notes and medical history

Complete psychosocial assessment and entrance review on each client

Develop an individualized service plan for each client

Conduct individual and group treatment planning sessions

Provide individual and group counseling sessions to identify and assess issues, goals and measurable objectives

Provide didactic and experiential education for clients

Maintain client records and document all changes and progress in clients behavior in accurate, chronological progress notes

Initiate and document contact with family members and other relevant parties

Ensure all documentation is maintained according to state licensure regulations

Document progression through the level system and in accordance to the level system

Complete discharge summaries

Work with the multidisciplinary team to ensure that the program in delivering a high quality treatment experience

Maintain ethical standards and continued professional growth

Maintain current licensure/certification including CES, CPR, First Aid, and Medication Management

Ensure client confidentiality in accordance with all federal, state, and local laws and regulations.

Qualifications for Position:

Counselors must have a masters degree in a behavioral health or related field and must be certified or licensed in Virginia (or under supervision to be licensed or certified). At least one year experience working in substance abuse/addiction treatment or related behavioral health program required.

Supervisory Relationships:

Counselors report to the Program Manager of the PHCC IOP and Outpatient Programs.

Skills and Knowledge Required:

Must have knowledge of chemical addiction treatment and of 12 Step recovery process. Must demonstrate empathy, compassion, and respect for clients in group, individual counseling, and daily interactions. Knowledge of Medication Assisted

Treatment helpful. Amount of travel required and any other special requirements: Position will usually require

minimal travel

Hours per day or week: 40 hours per week, Monday Friday, afternoons

Screens, evaluates and provides counseling for clients as indicated by interviews and assessment. Assessments include complete psychosocial evaluations, and risk assessments for behavioral health related issues. Develops. documents and presents identified problems and proposed treatment plans to individuals, groups and families. Identifies or develops sources for needed therapy and other support services. Uses a variety of modalities and techniques as appropriate for situations and clients. Prepares required reports and records. Informs appropriate points of contact of status, progress, duty limitations and ways to facilitate the treatment process. Works with associated staff from work, school, and treatment facilities to ensure accurate and proper handling of cases.
