Position Summary The Outpatient Dietician will work with the White Plains Hospital Family Health Center patients to improve their health and well-being through Medical Nutrition Therapy (MNT) Essential Functions and Responsibilities Includes the Following: 1. Understands and adheres to the WPH Performance Standards, Policies and Behaviors. 2.

The outpatient dietician will work with White Plains Hospital Family Health Center patients to improve their health and well-being. The outpatient dietician will provide Medical Nutrition Therapy (MNT) counseling to individuals. This entails creating nutrition care plans using the Nutrition Care Process to specify diet plans or goals for patients with varying medical diagnosis such as diabetes, chronic renal insufficiency, metabolic syndrome, heart disease and more.

The outpatient dietician may counsel individual patients or their families in a private or group setting. Other responsibilities may include: nutrition research and presentations, wellness classes, creating and maintaining nutrition materials. 3. Utilizes cost containment practices. 4.

Utilizes appropriate steps in the decision making process to include recognition and priority setting related to patient care. Addresses issues with appropriate actions. 5. Collaborates with the health care team. 6.

Integrates patient right into work practice.e 7. Utilizes and monitors methods of infection control. 8. Participates in performance Improvement initiatives. 9.

Performs other related duties as assigned. Education Experience Requirements Registered Dietitian licensed or certified where applicable. Minimum of 1 year experience preferred.

New graduates will be considered. The outpatient dietician position requires, at minimum, a bachelors degree in food and nutrition, dietetics or health related field, a completed CADE approved dietetic internship and passing of the Registration Examination for Dieticians. The position requires the dietician to be credentialed in good standing with the A.N.D.

Commission on Dietetic Registration. Preferred knowledge and experience with Electronic Medical Records. Core Competencies 1. 2.

Ability to conceptualize, produce and evaluate programs 4. Excellent written and communication skills 5.

Excellent Communication Skills (both verbal and written) 6.

Attention to Detail 7. Planning Organizing Physical/Mental Demands/Requirements Work Environment ï'· May be exposed to chemicals necessary to perform required tasks.

Any hazardous chemicals the employee may be exposed to are listed in the hospitals SDS (Safety Data Sheet) data base and may be accessed through the hospitals Intranet site (Employee Tools/SDS Access). A copy of the SDS data base can also be found at the hospital switchboard, saved on a disc. ï'· Position requires ability to move about freely at least 85 per cent of the time. ï'· Must be able to remain stationary for extended/prolonged periods ï'· Must be able to work in environment with continuous interruptions ï'· Must be able to adapt in a high stress and fast-paced environment with multiple interruptions ï'· Must be able to constantly operate computer, ï'· Position requires ability to concentrate on fine detail with interruptions and attend to tasks for more than sixty (60) minutes at a time. ï'· Individual must be able to understand and relate to theories behind several related concepts.
