Job Summary:

  • The OTP Case Manager role is part of the Case Management team that supports ETS' mission by providing engagement and intensive case management and care coordination services to patients engaged with OTP services and specialized support.

  • This role makes a critical difference at ETS by providing direct services such as outreach, mid-term, engagement, and supportive services for patients through intensive case management activities and collaboration with ETS partners, service providers, housing providers, and other community organizations..

  • The position is full-time onsite, the location is at ETS' Seattle Clinic facility, and the days and hours are Monday through Friday, 5 AM - 1:30 PM.

  • The pay range range for this position is $28.00 per hour - $30 per hour. (The annual pay equivalent is $58,240- $62,400 per year.). ETS strives to achieve pay equity and transparency as part of our goal to create an equitable workplace.

What you contribute to this role

  • Responsibilities:
  • Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.

  • Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.

  • Work with patients to develop individualized case management plans.

  • Develop rapport and a trusting relationship with patients through respectful outreach and patient-centered engagement practices.

  • Develop and implement, in collaboration with the patient, an Individualized Service Plan which addresses the patient's stated needs and priorities. Review and update this plan regularly to reflect progress or attainment of articulated goals and the emergence of new patient needs and to help the patient move toward the achievement of autonomy.

  • Assist patients in obtaining housing and maintaining housing stability.

  • Assist patients with establishing and maintaining primary medical care and addressing chronic health conditions.

  • Address patients' needs for mental health and facilitate engagement in appropriate supportive services.

  • Assist patients in achieving financial stability through access to entitlement programs, protective payee-ships, educational and vocational programs, and employment opportunities.

  • Develop and maintain productive working relationships with community programs which may support patients to provide effective coordination of care.

  • Provide services consistent with ETS program policies and procedures.

  • Update Case Management plan periodically to reflect movement toward or attainment of articulated goals and the emergence of new needs to help patient move toward autonomy.

  • Continually monitor and evaluate patient's progression through their case management plan and reach out to patient as needed.

  • Conduct individual case management sessions and document all patient contacts as required by federal, state, county, and ETS guidelines.

  • Required to complete prompt and accurate progress notations of all interactions with patients in assigned patient files.

  • Prepare reports including but not limited to outcomes, successes, etc.

  • Identify gaps and barriers in available community resources and advocate for systemic changes.

  • Attend program-related community, coalition and committee meetings as assigned.

  • Consistently maintain professional boundaries with patients and coworkers.

  • Act consistently to protect the confidentiality of all patients and the integrity of professional staff communication and behavior.

  • Adhere to all confidentiality requirements and guidelines outlined in the 42 CFR, Part 2 federal regulations pertaining to chemical dependency treatment records.

  • Conduct crisis intervention as needed.

  • Some local travel may be required on behalf of the agency.

Requirements

What you bring to this role

  • Qualifications:

Education and/or Relevant and Lived Experience (if applicable):

  • Bachelor's Degree in Social Sciences, ex: Social Work, Psychology and Sociology, etc.

  • Two (2) years in a Case Manager role preferred

Knowledge and Skills:

  • An understanding of racial justice and social equity and a commitment to helping create an equitable environment for all ETS clients and patients as well as fellow staff.

  • Ability to establish and maintain effective working relationships with clients, patients, and staff from a wide variety of ethnic, socioeconomic, and cultural backgrounds.

  • Understanding of harm reduction along with a demonstrated passion for serving individuals experiencing homelessness, substance use, and co-occurring disorders required. Street outreach experience is a plus.

  • Ability to be creative in developing and implementing effective approaches to chemical dependency treatment.

  • Ability to read and interpret general business correspondence, policies, and procedures, referral information, financial documentation, and applicable government regulations.

  • Strong interpersonal skills and verbal/ written communication skills.

  • Excellent organizational skills and ability to prioritize workload, work independently, and complete tasks timely and efficiently.

  • Dependable, able to work under pressure, receptive to change, willingness to learn, cooperative approach to problem solving.

  • Flexible team player, with excellent attention to detail.

  • Ability to maintain confidentiality and use discretion when handling highly sensitive information.

  • Ability to set boundaries, resolve conflict and de-escalate issues.

  • Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.

Additional Essential Information:

Physical Conditions and Requirements:

  • The employee may be exposed to illicit drug residues and fumes or other bio-hazardous materials when carrying out job functions. There is also potential for exposure to bloodborne pathogens. ETS will provide employees with appropriate training to limit the risk of exposure to bloodborne pathogens. Policies and procedures are in place addressing each item specifically.

  • The employee is regularly required to sit; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms and talk or hear; frequently required to stand, walk, and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.

  • The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color, and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
