Job Summary

Assesses patients then develop and initiate treatment plan based on the results of the evaluation. The evaluation may include measurement of physical, cognitive, perceptual and/or developmental skill as they relate to maximizing level of independence in the occupational performance areas along the continuum of life. Provide direct patient care. Documents findings, progress and instruction to patients and caregivers. Assists Level III occupational therapists and director with program development. Serves as resource to staff OT's. Provides training and guidance to students and interns. Supervises assistants and rehabilitation technicians with regards to individual patient care.

Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. He/she must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles. Minimum Qualifications

Education: Bachelors or degree in Occupational Therapy from an accredited university-based Occupational Therapy program

Licenses/Certifications: Texas Board of Occupational Therapy Examiners license

Experience/Knowledge/Skills:

  • Minimum of two (2) years of clinical experience required

  • Demonstrates ability and willingness to mentor/train staff or supervise clinical interns

  • Provides clinical leadership and demonstrates expertise for a particular patient type/group or enhances an existing clinical program for that particular patient type/group

  • Serves as a clinical resource for hospital and/or system or departmental committee/task force or initiates and/or provides leadership in an ongoing departmental interest/study group

  • Designs and implements original staff development program which facilitates the clinical competencies of other staff members

  • The position requires occasional to frequent lifting up to 75-100 pounds and carrying of up to 30 pounds; prolonged standing; frequent walking, stooping and squatting

  • Fine hand dexterity, eye-hand coordination, the ability to receive oral and written communication, and to communicate verbally and in writing are also necessary

  • The visual ability to monitor instruments, computers and equipment, recognize alphanumeric characters, distinguish colors and observe physical characteristics are essential to this position

Principal Accountabilities

  • Assumes responsibility of the care of the patient.

  • Performs complete, comprehensive evaluation of patients with a physician's referral.

  • Accurately interprets evaluation findings.

  • Completes a written plan of care, develops goals based on evaluation findings, and develops a comprehensive program to attain goals.

  • Implements an occupational therapy treatment program and provides advanced occupational therapy treatments.

  • Demonstrates competency in performing advanced occupational therapy skills.

  • Assesses patients periodically to monitor progress towards treatment goals, and alters goals, plan or program as indicated.

  • Supervises assistants and rehabilitation technicians with regards to patient care. Motivates and instructs patients/caregivers using appropriate methods.

  • Communicates effectively with patient/caregiver and professional colleagues including physicians and insurance companies.

  • Manages time effectively. Documents appropriately in the medical record and completes other documentation duties accurately and efficiently.

  • Serves as clinical resource to staff, hospital, community and/or systems or departmental committee/task force.

  • Acts as clinical instructor to OT and OTA interns.

  • Assists in program development within the department.

  • Assists in supervision of support staff.

  • Demonstrates expertise in treatment of a particular patient population.

  • Actively participates in organized community service/functions as a representative of the profession.

  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

In this position, you'll promote excellence in pharmaceutical care through the management of the medication use and safety process, identifying and preventing medication use errors, and resolving any medication use and safety related issues. Additionally performs all duties of a registered pharmacist as defined by the Hospital. Think you've got what it takes? Responsibilities * Ensures medications and patient care meet the highest standards * Ensures the appropriateness of all medication doses in reference to the patient's diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by a review of medication incidents * Accurately enters and/or verifies medication orders in computer as evidenced by a review of medication incidents * Ensures the medications that patient receives are correct as documented by a review of medication dispensing incidents * Ensures medication orders/prescriptions are processed and dispensed within established guidelines as evidenced by peer review and absence of medication incident/customer compliant * Follows aseptic technique and infection control in the preparation of sterile products. Insists technicians and other pharmacists practice aseptic technique and infection control. Completes annual re-certification and scores at least 90% on the aseptic technique/infection control skills and written exam as documented by a qualified observer * Participates in the education of patients, families, and/or caregivers. Documents education of patient/family or caregivers per procedure * Participates in daily quality/clinical review of patients profiles to ensure appropriate medication utilization and provides counseling as appropriate * Participates in the education of pharmacy students as evidenced by student agendas * Provides immunization and smoking cessation education to the patients' caregivers under developed physician-pharmacist protocols and documents appropriately evidenced by pharmacy computer system reports * Checks all means of communication including e-mail and mailbox every shift. Responds to pages and messages within departmental standards * Participates in hand-off communication as the on-coming and off-going pharmacist including verbal communication and any other established procedures * Acts as a liaison between the pharmacy, the nurses, and practitioners as evidenced by lack of complaints * Demonstrates positive and effective interpersonal skills when dealing with patients/families, visitors, peers, and other healthcare team members * Understands and follows OBRA '90 rules and regulations. Performs profile review and patient counseling as appropriate * Ensures work environment is clean, neat, and organized according to departmental standards * Ensures the smooth running of the pharmacy and acts as a role model for technicians and other pharmacists as evidenced by lack of complaints, medication incident reports, and peer review * Maintains patient confidentiality as documented by lack of complaints and peer review Qualifications * Degree in Pharmacy or Pharm. D.
