What you'll do:

  • Perform excellent comprehensive eye exams in accordance with Warby Parker's protocol and standards of care

  • Ensure flawless accuracy in taking measurements and diagnosing ocular conditions

  • Explain eye health, eyewear, and prescription terminology to patients

  • Demonstrate exceptional product knowledge and effectively communicate features and benefits to patients

  • Act as a representative for our brand and help educate and excite others to establish Warby Parker as a leading vision service provider

While there are many goals of this program, the overriding goal is to improve the quality of life for the member by reducing unnecessary hospitalizations and emergency rooms visits. Signify Health is focused on improving the quality of life for Health Plan Members through effective care coordination and best in class care management. The Signify Health Care Management Program is for health plan members residing in long term or supportive care facilities. Qualifications: Education/Licensing Requirements: * APRN with license in good standing in the state where work is being performed * Active DEA License * One or more years as NP Preferred * Family Medicine, Geriatrics, and Psychiatric Nurse Practitioners are encouraged to apply * Nurse Practitioners fluent in Spanish are encouraged to apply Essential Skills/Experience: * Home Care or Long-Term Care experience, preferred * Experience in utilization review, concurrent review and/or risk management a plus * Bi or multilingual ability a plus * Minimum of 2 years' clinical experience working with complex adult populations * Understanding and ability to apply principals of Care Management and individualized care planning * Prior experience in Family Medicine, Geriatrics, and Psychiatry preferred * Understanding of issues related to communication, cognitive or other barriers * Strong critical thinking skills * Strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner * Strong computer skills with working knowledge of Microsoft Office and various computer software programs * Able to work in-person or remote * Ability to effectively present information and respond to questions from members, providers, team members and managers Essential Characteristics: * Self-directed and organized * Discrete/ability to maintain confidentiality * Team player * Detail-oriented with a focus on accuracy * Sense of urgency * Adaptable * Ability to execute multiple projects simultaneously in a deadline-driven environment * Ability to work well independently and takes ownership of his/her work product * Comfortable working in a high-growth, evolving environment * Manage time effectively * Respond well to coaching and supervision Essential Job Responsibilities: * Conducts regular in person follow-up with members and/or caregivers, as well as appropriate medical staff based on care needs identified during clinical assessments * Partners with Signify Health, the health plan and providers to help coordinate both internal and external resources to meet the member's identified needs and to manage the member's plan of care * Quickly responds to urgent/emergent conditions and hospital admissions to ensure that the member receives the appropriate care in the appropriate setting * Assists with improvement to work processes and commits to a focus on continuous improvement of workflow processes and procedures * Attends meetings, training sessions and participates on committees as requested * Possesses a strong knowledge of clinical procedures, standards and quality control checks * Possesses a strong knowledge of medical conditions, interventions, treatment and medications * Provides members, caregivers and facility education * Monitors the quality of member's care and updates plan of care * Identifies / implements cost-effective approaches and interventions * Performs other related duties and responsibilities as assigned Additional Job Responsibilities: * Participate in staff meetings, conference calls and other meetings, as needed * Complete reports as requested by leadership * Assist/coach/coordinate with other team members * Attend training sessions to acquire/enhance skills related to programs offered * Proficient in Microsoft Office products * Performs other incidental and related duties as required Working Conditions: * Fast-paced environment * Requires working at a desk to use a phone and computer * Requires travel within the region, distance varies by area * Use office equipment and machinery effectively * Work effectively with frequent interruptions * May require occasional overtime to meet project deadlines Signify Health Chicago IL
