An Independent Doctor of Optometry affiliated with LensCrafters (Luxottica) seeks an Associate Optometrist. You will work within the practice of a LensCrafters Subleasing Optometrist as an employee or contracted doctor.

As a private Independent practice, Luxottica operates the optical dispensary while the Affiliated Doctor operates the Doctors office and provides high quality optometric care and service to patients. Working as an Associate Doctor of Optometry within the practice, the terms of agreement, practice model, pay and hours are all negotiated directly between you and the Subleasing doctor.

This is a place where you can stay way ahead as you do your life's best work.(sm) The Case Management Social Worker of ProHealth Physicians assists the Medical Management Team and physicians in the on-going monitoring of complex patients. This position is Monday-Friday with standard office hours. Primary Responsibilities: * Identify psychosocial needs and interventions for patient's condition * Proactively help prevent patient ER and acute admissions when possible by emphasizing prevention of exacerbation's and complications utilizing cost-effective evidence-based practice guidelines and social service interventions including patient self-management * Facilitate the improvement of patient compliance via telephonic follow-up and other alternative or non-traditional methods * Help to improve patient satisfaction which includes supporting the physician/patient relationship and plan of care * Assist in maximizing patient access to quality health care via coordination with PCP's, Specialists, social services, home health, health education, and laboratory services * Develop and maintain social service program, drafting supporting policies, procedures, and guidelines for the program * Telephonically assess patient's psychosocial status * Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems * Works closely with all members of the healthcare team in development of the plan of treatment based on psychosocial assessment and accepted social work interventions * Provide social work services including, short-term counseling, community resource planning, and crisis intervention * Acts as a resource person for other members of the healthcare team * Develops and maintains through knowledge of referral resources throughout the service area * Report to OCNCT Case Management Nurse variances in patient's condition, medications, or treatment * Develop and maintain social service program, drafting supporting policies, procedures, and guidelines for the program * Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems * Works closely with all members of the healthcare team in development of the plan of treatment based on psychosocial assessment and accepted social work interventions * Provide social work services including, short-term counseling, community resource planning, and crisis intervention * Acts as a resource person for other members of the healthcare team * Develops and maintains through knowledge of referral resources throughout the service area * Report to OCNCT Case Management Nurse variances in patient's condition, medications, or treatment * Telephonically monitor patient on a regular basis in regards to psychosocial needs, education, and compliance with treatment plan * Maintains up to date knowledge of Medicare, Medicaid, and other third party reimbursement sources * Provides positive customer relations in dealing with patients, families, physicians, team members, hospital departments, referral resources, service agencies and others * Refer to health education and other service departments as necessary * Complete all required documentation in a timely manner according to department policy * Refer patient to outside community resources, classes, education, etc. as appropriate * Case closure as appropriate * Collect data necessary for OCNCT Case Management reports/QI studies * Attend OCNCT Case Management meeting and trainings as appropriate * Adheres to all department policies * Other duties as needed You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
