The Optical Imaging Index Clerk is responsible for indexing inpatient and outpatient chart documents in the optical imaging system according to department policies and procedures.
JOB DUTIES AND RESPONSIBILITIES:
Responsible for retrieving medical record documents to be scanned from appropriate patient care units.
Responsible for prep, scan and indexing all inpatient and outpatient chart documents in the optical imaging system. Adheres to all policies and procedures associated with indexing chart documents.
Responsible for meeting department quality standards for indexing, 95% accuracy rate.
Responsible for meeting department standard of 24 hour TAT.
Responsible for maintaining productivity data sheet weekly.
Assists in training new personnel as needed.
May performs QA on prep, scan and index functions.
May have to travel to other locations as needed.
Abstracting information into patient charts as needed for VBI reporting.
Assign deficiencies to providers.
Monitor outstanding signatures for providers and report back to managers.
Create in basket message to office staff for documents.
Other related duties as assigned.
PHYSICAL AND SENSORY REQUIREMENTS:
Sitting for up to 7 hours per day, 3 hours at a time. May require occasional standing/walking for up to 1 hour per day, 30 minutes at a time.
Frequently uses fingers for data entry. Frequent use of hands and fingers as it relates to holding patient records. Uses upper extremities to lift and/or carry up to 10 pounds.
Hearing as it relates to normal hearing. Seeing as it relates to general vision, near vision, peripheral vision and visual monotony. May require stooping, bending and/or reaching above shoulder level.
High school graduate with medical terminology and word processing course preferred. Work experience in a medical record or office environment with
appropriate knowledge base highly considered. Accredited Record Technician (RHIT) and or graduate of two-year medical secretarial program preferred.
TRAINING AND EXPERIENCE:
Minimum of 1 year in the field.
