This is an Other Personnel Service (OPS) position working 40 hours per week with the Department of Health in Lee County.

This OPS Registered Nurse position is under the supervision of the Senior Community Health Nursing Supervisor with the Florida Department of Health in Lee County. The incumbent provides public health nursing services for clients in FDOH -Lee clinics. The incumbent must maintain a current working knowledge of the Department of Health's STD/HIV program and policies. This position is classified as a sensitive position. Incumbent handles protected health information on a need to know basis following Department of Health, HIPAA and Information Security and privacy policy. Employee abides by State/Federal laws governing health information. Responsible to maintain confidentiality, security, privacy and integrity of protected health information. Refers and utilizes DOH 50-10. Incumbent will have access to records containing social security numbers in the performance of job duties. This position has access to the following information sets: Medical Records, Laboratory Reports, Immunizations, FLSHOTS, and the electronic health records system. Incumbent may be required to work during or beyond normal work hours or days in the event of an emergency.

CLINIC SERVICES:

Promotes and maintains health of individuals, families, and the community through appropriate preventative and rehabilitative measures. Provides comprehensive nursing in all FDOH Lee clinics and community settings as warranted. These services include: administration of immunizations based on patient's immunization status, current immunization guidelines, vaccination schedules, and statutory requirements, reproductive health services, counseling/testing, health screening, health education, referrals and follow-up of the referrals, timely and thorough documentation in the electronic health records system using the SOAP note format, and collection of laboratory specimens. Provides treatment of STIs to PrEP, nPEP, and Test and Treat patients based on provider orders and protocols.

SUPPORT SERVICES:

Provides case management services to eligible STI, Pre-Exposure Prophylaxis (PrEP), Test and Treat, and Post Exposure Prophylaxis (nPEP) clients, including scheduling appointments. Conducts client centered PrEP and HIV/STD risk reduction education and adherence counseling services with a focus on client acquisition, engagement, and retention. Conducts care planning activities, such as scheduling appointments; monitors appointment adherence, identifies service gaps; recommends case closure for patients in partnership with medical provider, and HIV/STD staff. Completes medication coverage/patient assistance applications. Coordinates care with DIS and HIVcare team.

Acts as a liaison between HIV-infected clients and community agencies to ensure continuity of care. Assists clients with completion of Department of Health Consent/Authorization forms for release of information, as needed.

Maintains comprehensive quality records in FLSHOTS and the electronic health records system, collects and reports pertinent data to advanced practice registered nurses and physicians; provides appropriate case follow-up; participates in case conferences; confers with other professionals; formulates and evaluates care plans, assists in record reviews; recognizes and understands the changes in the community and nursing practice which affects community health unit programs/plans; serves as staff advisor to student affiliates and new staff nurses.

Assists with health fairs, community organizations, and specific public health events as requested.

Prepares reports or other required data within specified time frames. Attends pertinent conferences as requested or permitted.

Participates in staff development, team meetings, in-services, webinars, and continuing education opportunities. Participates in QA/QI meetings and activities.

Prepares biweekly leave/attendance records and daily employee activity records (EARs).

Performs other duties as assigned.

Licensure/registration/certification requirements (If applicable, list the appropriate Florida Statute or federal regulation cite):

  • Licensure as Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27.

Other job-related requirements for this position:

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

  • Incumbent must possess a valid Florida Driver License and have a motor vehicle to carry out these duties.

  • Selected immunizations or titers and/or periodic screening for tuberculosis may be recommended for this position.

  • Must be willing to undergo FIT Testing

  • Must have current CPR certification or obtain such certification during employment.

  • This position is authorized to transport confidential information outside of established DOH-Lee facilities and must comply with parameters defined in DOHP 50-10.

  • This position has the responsibility of being the Information Custodian for the PrEP and nPEP information set and will perform related duties as defined in DOHP 50-10.
