Your Specific Responsibilities:

Assist in the completion of annual reports and foundational documents (Community Health Needs Assessment, Community Health Improvement Plan, Strategic Plan, Performance Management / Quality Improvement Plan, Workforce Development Plan, and Emergency Operations Plan) revisions by deadlines set by Central Office.

Research and conduct training courses for employees and/or community members.

Track the implementation of the Community Health Improvement Plan, Strategic Plan, Performance Management / Quality Improvement Plan, Minority Health Plan, and Workforce Development Plan.

Collect data to update the Performance Management Dashboard.

Attend community meetings and engage in discussions aimed at improving population health.

Engage employees in formal quality improvement projects.

Collaborate with community partners to improve health in the community (participate in committees, share data and information, connect people to resources).

Required Knowledge, Skills, and Abilities:

Qualifications:

Bachelor's degree or 4 years' experience in performance management and/or quality improvement.
