Pharmacist - 2418672

Roles & Responsibilities:

The Pharmacist Level 1 serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.

Duties and Responsibilities:

1.Dispenses medications accurately and in a timely fashion
2.Resolves questions regarding formulary/ and non-formulary medications, knowledge of and follows policies and procedures
3.Provides dosing & therapeutic recommendations that take patient age/ kinetic variables based on literature
4.Performs clinical chart reviews on a scheduled basis
5.Performs Patient Care Unit inspections on a scheduled and timely basis
6.Prepares parenteral medications using sterile aseptic technique
7.Maintains adequate medication supply in the Pharmacy area and informs supervisor of impending shortages
8.Intervenes and documents interventions to correct/ improve pharmacotherapy
9.Reports adverse drug events and medication errors occurring in area serviced
10. Takes responsibility for the operation of the assigned Pharmacy area and activities of all supportive personnel
11. Adheres to Hospital's dress code
12. Maintains an acceptable attendance record
13. Keeps up to date by reading and attending workshops, conferences and C.E. programs to maintain professional competency
14. Perform chart reviews, looking for appropriateness of drug therapy for the disease or diseases being treated as per department requirements.
15. Responsible for medication reconciliation as per department requirements.
16. Performs patient education, including but not limited to, proper dose as prescribed, taking meds on time, side effects if any, interaction with other drugs patient might be taking, all as per department requirements.
17. Completes PEAK training and educational requirements as per department and/or hospital requirements.

Requirements:

Education:

Graduate from an accredited College of Pharmacy (ACPE).

Experience:
0-2 years as licensed Pharmacist

REQ# 2518918 The division of Neuro-Oncology seeks a board certified Nurse Practitioner who will work as an integral part of the inpatient neuro-oncology consult team. You will work in collaboration with neuro-oncologists, oncology fellows, neurology residents to provide recommendations in management of patients hospitalized with brain tumors and neurological complication of cancer. The job will require obtaining comprehensive histories, physicals and neurological examinations, communicate recommendations including work-up, chemotherapy treatment plans, and perform specialized procedures (with training to assist in lumbar puncture, intra-thecal chemo-therapies). Your will utilize your specialized knowledge to counsel patient during new diagnosis of cancer, hospitalization for chemotherapy, enhance continuity of care to the outpatient oncology team, and prevent patient care problems. Roles & Responsibilities: Provides acute/primary health care in accordance with written policies approved by nursing and medical administration. Responsible for patient care as outlined within collaborative practice agreement. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. * Assesses the health status of patients/families by means of comprehensive health history and physical examination. * Initiates selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated area. * Provides teaching and counseling in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. * Documents findings and plan of care in the patient's medical record. * Consults with physician as established in the collaborative practice agreement. * Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months * Advocates for patients/families and supports patients' rights. Requirements: * RN / Registered Nurse License * CNP / Certified Nurse Practitioner License (A Nurse Practitioner is certified under section 6910 of the New York State Education Law and licensed as a Registered Nurse.) * Previous nursing experience in neurology, oncology or acute care is preferred. A minimum of 1-2 years of RN/NP experience preferred. * Master's degree program designed to prepare a registered nurse to provide acute/primary health care services or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate. * Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. * Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and federal Laws relating to prescriptions and record keeping (3-hour course). * Has a collaborative practice agreement with a physician and designated protocols, both filed with the New York State Department of Education. * Previous progressive experience relevant to the area of clinical practice. * Professional certification by relevant professional organization preferred.
