Job Summary: Reporting to the Manager of the Oncology Navigation team, the Cancer Institute Nurse Navigator serves as a single point of contact for patients with cancer, their family members, caregivers, and referring physicians to access disease-specific cancer care services at Lehigh Valley Health Network.

The oncology-experienced Nurse Navigator is responsible for managing the care of identified patients with cancer in collaboration with a team of healthcare professionals to ensure timely, high quality care across the cancer care continuum within an accountable care framework. The cancer care continuum is defined as the time of diagnosis to survivorship or hospice. The Cancer Institute Nurse Navigator works with her assigned patient population to assure seamless access to oncology care by frequently assessing medical and psychosocial patient needs, and assuring treatment compliance.

Along with the physicians and healthcare team, the Nurse Navigator answers healthcare-related questions, refers oncology patients to resources both within the Health Network and in the community, and provides education. The Nurse Navigator works independently to create a work schedule according to patient acuity and needs, accompanying patients to Specialist appointments as needed to assure compliance and understanding of the treatment plan. The Nurse Navigator is the leader of disease-specific Multidisciplinary Conferences, presents MDC patients at Oncology Tumor Boards, and leads professional meetings and case conferences.

The Nurse Navigator also participates in Cancer Institute and community outreach activities, promotes Cancer Support Services, and educates the community in Cancer Institute and oncology-related topics. The Nurse Navigator demonstrates excellent customer service, written and oral communication skills, and presentation skills. Excellent critical thinking skills required.

Minimum Requirements: graduate of an accredited nursing program (RN) 2 to 3 years RN Oncology experience required Excellent communication skills; ability to advocate for patients with all members of the health care team; Good organizational and documentation skills; Ability to manage a clinical caseload of patients with cancer.

Preferred Qualifications:
BSN 5 years Licensure and Certifications: PA RN BLS required;PA RN Licensure;OCN or OCN certification required within 2 years of eligibility BSN preferred 5 years oncology experience preferred Bi-lingual preferred (Spanish) #

The Registered Nurse (RN) delivers nursing care for patients across their life span, and through the nursing process, assesses, plans, implements, and documents the effects of care. Works cooperatively with ancillary, nursing and other patient-team personnel to maintain standards for professional nursing practice. JOB DUTIES AND RESPONSIBILITIES: * Responsible for overseeing the gathering of appropriate baseline intake information (from patients and referring physician offices) for working lung group program (charts, medical reports, clinical office notes, medications, pathology reports, radiology reports, and demographics). Establishes priorities based on patient assessment. * Provides multidisciplinary team with appropriate information and patient summaries. * Attends all assigned working lung conferences, facilitating the conference with physician leader. * Disseminates Lung Working Group recommendations within one day of meeting and disseminates appropriate documentation to each physician involved in patent's care, including referring physician. * Provides individualized patient/family information, support, education and resources regarding lung cancer, diagnostic testing, procedures and surgeries. * Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment. Empowers patient/family to participate in the plan of care, documents compliance and concerns, monitors patient progress, consults with physicians and other members of the health care team at agreed upon intervals. * Appropriately refers/assists with coordination of appointments for consultations, physician referrals, and community resources as related to the lung cancer program. * Interacts with administration, manages and facilitates program improvement, keeps team up to date with program and monitors progress in achieving desired outcomes. * Manages the cross continuum care of multidisciplinary patients to provide high quality of care in an effective and efficient manner with close attention to service excellence. * Acts as network-wide lung oncology resource nurse and program coordinator of lung working group. * Identifies patient and/or provider needs and ensures delivery of quality services to patients. * Collaborates with Network Clinical Studies Coordinator in clinical trials enrollment and data collection. Reviews and identifies appropriate cases for clinical trial eligibility with the study coordinator. * Other related duties as assigned. PHYSICAL AND SENSORY REQUIREMENTS: Sit up to 7 hours per day; 3 hours at a time. Stand for up to 3 hours per day; 2 hours at a time. Walk 1 hour per day; 10 minutes at a time. Frequently lifting, carrying and pushing objects up to 10 pounds. Rarely lifting, carrying and pushing objects up to 75 pounds. Frequently stooping and bending. Frequently reaching above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, high and low frequencies. Must be able to see as it relates to general near, far, color and peripheral vision. Must be able to speak as it relates to normal conversation and projection as it relates to lectures and presentations. EDUCATION: Bachelor's Degree required, OCN preferred. Registered Professional Nurse with current license to practice in the State of Pennsylvania. TRAINING AND EXPERIENCE: Three to five years Oncology Nursing experience required.
