Position Responsibilities:

  • Demonstrates pharmacy clinical service expertise; is able to to focus and serve as a lead in the provision of pharmaceutical care to patients within the assigned specialties.

  • Develops and implements new pharmacy practice programs and initiatives to further promote the safe and effective use of medications.

  • Identifies, prevents, assesses and resolves medication-related problems.

  • Builds effective relationships with other healthcare professionals and appropriate manufacturer representatives.

  • Promotes safe, efficacious and cost-effective medication therapy by participating in the provision and documentation of clinical pharmacy services and execution of medication utilization initiatives.

  • Promotes clinically rational drug therapy and sound pharmaceutical care through the development and implementation of new (or expansion of existing) pharmacy practice programs and practices.

  • Reviews drug therapy policies and practices to comply with safe and effective use of medications and ensure patient safety.

  • Evaluates, interprets, compounds and dispenses medications and other pharmaceutical supplies according to written orders/prescriptions issued by an authorized prescriber.

  • Supports Pharmacy management in overseeing Pharmacist and Technician's performance; leads their compliance with organizational and departmental policies and procedures. Provides performance feedback as required.

  • Ensures all patient care provided meets or exceeds all regulations, statutes and JCAHO standards.

  • Analyzes and reports pharmacy interventions and adverse drug reactions.

  • Identifies and analyzes prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.

  • Is proficient in the use of provided system solutions to generate required information or trackings.

  • Provides critical input to achieve and maintain appropriate and cost-effective inventory levels.

  • Makes sure expired medication is identified and segregated on a timely basis.

  • Implements and participates in PI program as required by the Medical Center.

  • Documents competency in clinical skills, at least annually.

  • Organizes and / or participate in staff meetings as required.

  • Coordinate with nursing and other ancillary staff to address specific clinical as well as EPIC issues to mitigate delays for patients

  • Maintain all additions and alterations for the oncology library in the smart pumps

  • Create all new EPIC oncology builds and alterations as new agents are introduced or when indications change or are added.

  • Assists manager as the CCC pharmacy department EPIC super-user and clinical trainer for all new hires.

  • Serves as preceptor for any on-coming students (PharmD candidates) seeking experience in pharmaceutical oncology.

Qualifications:

  • Registered Pharmacist in the State of Florida. Certification by Board of Pharmaceutical Specialties is strongly preferred.

  • B.S. or Pharm.D. from an ACPE-accredited School of Pharmacy. Masters Degree preferred.

  • Completion of a 1-year pharmacy practice residency, specialty residency and/or fellowship, or at least three years of pharmacy experience is stongly preferred.
