TITLE: On Track Coordinator

QUALIFICATIONS:

1.State of Connecticut certification in School Counseling (068) or School Social Worker (071).

2.Master's degree required.

3.Five years of successful teaching experience preferred but not required.

4.Strong interpersonal, communication, and organizational skills and ability to work collaboratively with administrators, staff, community partners, students, and families in the provision of high-quality services and resources.

5.Ability to multitask, prioritize, problem solve, organize work, meet schedules and deadlines, follow directions and take initiative.

6.Maintains a strong belief in the East Hartford Public School district's core values and overall educational model.

7.Such alternatives to the above qualifications as the Board of Education may find appropriate and acceptable.

REPORTS TO: Department Head of Guidance and/or Principal

JOB GOAL: The On-Track Coordinator position at East Hartford High School (EHHS) is an extraordinary opportunity for a professional with planning, communication, coordination and event-management experience to work closely with 60 "summer scholars" (at risk) students who are entering the ninth grade. He/she will work closely across all academic teams in ninth grade to design, plan and execute individual student success plans (SSPs).

PERFORMANCE RESPONSIBILITIES:

  • Assesses student needs and progress and develops and implements strategic plans for each student on caseload.

  • Participates in meetings with 8th grade team members to identify students in need of support and develops 8th to 9th grade matriculation strategies and programs.

  • Creates a positive, achievement-oriented and structured learning environment that excites and engages students.

  • Motivates students to realize high academic and behavioral standards.

  • Engages students on caseload on a regular basis, including daily check-ins (at the start of the school day), during class time (teaching Freshman Seminar sections), weekly meetings (review attendance, academics and behavior data), and quarterly Scholar Hour protocols.

  • Builds community by investing families in the child's academic success.

  • Utilizes student data to drive instruction and interventions

  • Works with administrators and staff in compiling staff/student schedule requests.

  • Plans and implements programs and interventions to achieve established objectives and goals.

  • Helps students develop positive self-concepts.

  • Counsels students related to academic and/or behavioral goals.

  • Facilitates the development of student independence.

  • Provides data for grant evaluations as requested.

  • Supervises students and/or groups in transitional activities.

  • Collaborates with teachers, supervisors and school teams to improve instructional strategies.

  • Conducts individual student needs assessments in order to identify trends and the need for specific targeted interventions.

  • Conducts and review interventions with students and track progress.

  • Monitors student progress using the RISE Data Dashboard and/or PowerSchool and utilize analytic capabilities.

  • Facilitates on track data meetings.

  • Communicates with students, family members, school personnel and members of the community.

  • Establishes and expands community partnerships and family/school connections.

  • Promotes a positive learning environment.

  • Facilitates the cooperative involvement of parents and community in the educational process.

  • Cooperates in the ongoing studies of curriculum and school building needs and participates in the educational planning of school facilities.

  • Participates in professional activities such as workshops, conferences, parent/teacher meetings and activities of other pertinent professional organizations.

  • Attends all professional development, team planning and data analysis meetings.

  • Reports student data as directed by administrator(s).

  • Performs such record-keeping functions as the Principal(s) may direct.

  • Creates presentations as requested by Principal or supervisor.

  • Assumes such other functions as may be determined.

TERMS OF EMPLOYMENT:

Salary and work year to be established by the collective bargaining agreement between the Board of Education and EHEA.

EVALUATION:

Performance of this job will be evaluated in accordance with provisions of the Board's policy on Evaluation of Certified Personnel.
