On-Site Snf-Based Occupational Therapist (8 Week Contract, Asap Start) - Galax, VA

We have a phenomenal Occupational Therapist opportunity at one of our partner Skilled Nursing Facilities in Galax, VA for a 8 week contract, starting ASAP.

You will have the chance to positively impact the lives of patients by helping them regain independence and improve their ability to perform daily activities. Your expertise in assessing functional abilities, developing personalized treatment plans, and implementing therapeutic interventions will play a vital role in facilitating patients' rehabilitation and enhancing their overall quality of life.

Key Responsibilities:

• Conduct comprehensive assessments to evaluate patients' functional abilities, including motor skills, cognition, and activities of daily living (ADLs).
• Develop and implement individualized treatment plans tailored to patients' specific needs and goals, focusing on rehabilitation and functional independence.
• Administer therapeutic interventions and activities to improve patients' motor control, coordination, strength, and endurance, utilizing evidence-based techniques.
• Collaborate with interdisciplinary teams, including physicians, nurses, physical therapists, and speech-language pathologists, to ensure coordinated and comprehensive patient care.
• Monitor and document patients' progress, adjusting treatment plans as necessary and maintaining accurate records in compliance with regulatory standards.
• Educate patients, families, and caregivers on strategies for promoting independence, safety, and effective home exercise programs.
• Participate in discharge planning and transition of care processes, providing recommendations for continued therapy services and appropriate equipment or modifications.
• Stay current with advances in occupational therapy practice through participation in professional development activities, continuing education, and research.

Requirements:

• Valid license as an Occupational Therapist in the state where facility is located.
• Degree in Occupational Therapy from an accredited program.
• Previous experience working in a medical setting.
