The Social Worker is responsible for providing high quality services to the patient and the patient's family in order to promote optimal social and emotional functioning. These services include direct service to patients and their families both on an inpatient and outpatient basis. The Social Worker coordinates options and services to meet the patients' needs through collaboration and consultation with physicians and other healthcare personnel.

On-call social services are provided primarily to patients in the Emergency Department; however, the Medical, Surgical, ICU, Pediatrics and Obstetrics may also require the services of an on-call worker.

EDUCATION: Bachelor's with major in social work or related field with equivalent in education, training, or experience and documented mental health training or experience.

EXPERIENCE: Documented experience providing direct service to patients.

Knowledge of the mental health code. Knowledge of normal characteristics of growth and development throughout the span of life.

SKILLS: Demonstrates ability to work productively and effectively in a timely manner, ability to function well as a member of a multidisciplinary team, and ability to represent the program well in interactions both inside and outside the host hospital.

The Team Lead must possess the skill set to perform the SS functions; monitoring, educating and coaching the SS staff in skills related to assessment, planning, implementing, evaluation, progression of care, and processes that promotes performance in an efficient/effective manner. These functions include direct service to patients and their families both on an inpatient and outpatient basis. The Team Lead is responsible for demonstrating a positive and professional behavior, and professional communication with the health care team, patients, families and co-workers, as they work together to coordinate and collaborate to meet the needs of the patient in the hospital and in the outpatient settings. The Team Lead is responsible for collecting and reviewing data as assigned, identifying areas of improvement, coordinating standard work, promoting the Lean process, and acts as a resource for Mental Health Laws, psychiatric services, discharge planning, and Advance Directives. EDUCATION: Master's Degree in social work from a recognized school of social work. EXPERIENCE: Three or more years of documented experience providing all aspects of social services to patients (e.g. Mental Health laws, psycho-social assessments, discharge planning, outpatient services) with documented experience in acute care setting providing direct service to patients. Knowledge of normal characteristics of growth and development throughout the span of life. SKILLS: Demonstrates comprehensive clinical skills, ability to work productively and effectively, ability to function well as a member of a multidisciplinary team, and ability to represent the program well in interactions both inside and outside the hospital.
